Summary:
· We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our mission-driven healthcare team.
· In this role, you’ll deliver high-quality nursing care, support treatment plans, and collaborate with physicians and other healthcare professionals to achieve the best outcomes for our patients— many of whom are from underserved communities.
Responsibilities:
o Bring a “ONE TEAM” mindset each day.
o Provide direct patient care within your scope of practice, following established protocols.
o Patient triage for same-day urgent and post ER/IP admissions & Patient contact management of post ER and IP admissions.
o Direct Patient education, i.e., medication, post-procedure directives, pre-procedure preparations, changes to clinical procedures, or physician orders.
o Supplemental facilitation of laboratory requests and clinical encounter processing.
o Assistance with clinical procedures as directed by licensed practitioners.
o Collaborate with the care team to develop and carry out patient-centered care plans.
o Daily huddle participation and preparation.
o Chart prep oversight and coordination.
o Clinical inbox management- telephone encounters, pharmacy requests.
o Administer medications, injections, and treatments as prescribed.
o Monitor and document patient conditions, reporting changes promptly.
o Support providers during assessments and procedures; maintain accurate EMR documentation.
o Educate patients and families on medications, care plans, and healthy lifestyle choices.
o Maintain a safe, clean, and organized care environment in compliance with infection control standards.
o Offer guidance on health maintenance, disease prevention, and case management when needed.
o Quality Initiatives Management & documentation control.
o EDAPS & HEDIS quality Gap management.
o Coordination with Quality Team Nurses and Clinical Team to manage center initiatives and projects.
o Utilization management tasks for the center.
Required Qualifications:
o Current LPN license.
o Minimum 2 years of direct clinical experience (e.g., triage, EKGs, phlebotomy, medication administration).
o Experience with Electronic Medical Records (EMR).
o Strong teamwork skills and a high level of engagement.
o Ability to work effectively in a fast-paced, high-volume environment.
o Passion for improving patient experiences and outcomes.
Preferred Qualifications:
o Bilingual skills.
o Knowledge of HEDIS measures.
o Familiarity with state-specific primary care requirements.
o Understanding of clinical operations.

Pay: $28.00 – $30.75 per hour
