As a Physician Assistant, or Nurse Practitioner, you’ll be a vital member of our Interdisciplinary Team (IDT), providing compassionate primary care and helping create personalized care plans that support our participants’ physical, emotional, and social well-being.

What You’ll Do

• Provide comprehensive primary care to participants
• Complete assessments, evaluations, and care planning
• Collaborate closely with nurses, therapists, social workers, and caregivers
• Manage acute and chronic health conditions
• Coordinate nursing home care and discharge planning
• Order and review labs, diagnostics, and treatments
• Educate participants and families on health management
• Build lasting relationships with seniors and their caregivers

We’re Looking For

Physician Assistant

• Graduate of an Accredited Physician Assistant Program
• Current North Carolina Certification
• DEA Registration

Nurse Practitioner

• Current RN and NP Licensure
• Prescriptive Authority Preferred
• DEA Registration

For All Candidates

• CPR Certification – will provide
• Minimum one year working with the frail or elderly population
• Two years of geriatric experience preferred
• Passion for serving older adults
• Team-oriented mindset
• Commitment to person-centered care
