Full Job Description
**Summary**
This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval – award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
Duties
* Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice
* Develops and implements an individualized plan to achieve optimal population health and disease management.- Demonstrates leadership and works collaboratively with the interdisciplinary team
* Orders and interprets labs – tests – and diagnostic studies
* Prescribes pharmacologic and non-pharmacologic interventions
* Initiates referrals and consults to specialty services
* Provides education to patients and their families
* Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes
