Independent Contractor Agreement:

Mobile Wound Care Specialist Subcontractor (NP, PA, MD, or DO)

Position Type: 1099-NEC Independent SubcontractorCompensation: Percentage of Net Collections

About Us:Multispecialty Group providing comprehensive on-site care to Skilled Nursing (SNF), Assisted Living (ALF), and Residential Care Facilities. Based in Springfield, MO, we are expanding omnidirectionally and seeking dedicated wound care partners to enhance healing outcomes and the quality of life for our senior residents

.Option 1: The Independent Entity Model (High Autonomy)

1. Position OverviewThe Contractor (an independent clinical business entity) will provide mobile diagnostic and surgical/non-surgical wound management services to residents at facilities managed by our company. As a subcontractor, the Contractor operates as a separate business and is responsible for all clinical outcomes, specialized bedside equipment, and professional overhead.

2. Scope of Services (The “Result”)

  • Clinical Care: Contractor provides comprehensive wound assessments, sharp debridement, chemical cauterization, and management of pressure ulcers, diabetic foot ulcers, and venous stasis ulcers.
  • Regulatory Compliance: Contractor ensures all wounds are staged, measured, and documented weekly to satisfy state nursing home mandates (F-Tag compliance) and medical necessity.
  • Documentation: Contractor is responsible for accurate clinical charting (including photo documentation) and providing the necessary ICD-10 and CPT codes (e.g., 11042, 97597) for insurance billing to our Company.

3. Operational Autonomy (The “Control”)

  • Methods: Contractor has absolute clinical autonomy. The Company does not supervise or direct clinical techniques, debridement methods, or wound care treatment plans.
  • Scheduling: Contractor coordinates directly with the nursing facilities to establish their own visiting hours and service dates.
  • Tools/Equipment: Contractor must provide all necessary mobile wound care instruments (debridement kits, specialized lighting, measurement tools) and maintain said equipment at their own expense.

4. Compensation & Financial Risk (The “Collections” Model)

  • Commission Rate: Contractor shall be paid [Percentage]% of Net Collections (actual funds received from insurance/private pay) for services rendered.
  • Payment Cycle: Payments remitted within [Number] days of the close of each month, following receipt of funds from third-party payers.
  • No Minimum Guarantee: Compensation is strictly based on successful collections. No hourly or daily minimum wage is provided.
  • Expenses: Contractor is responsible for self-employment taxes, professional liability insurance, and state licensing fees.
  • Administrative Support: Credentialing and Billing will be through our company.
  • Supplies/Fees: Limited medical consumables (sterile gauze, saline, topical dressings) and flat travel fees/mileage reimbursements will be provided by the Company.

5. Qualifications & Requirements

  • Entity Status: Must operate as a registered business entity (LLC, PLLC, or P.C.) with an active EIN.
  • Licensure: Active MO Medical (MD/DO), NP, or PA License with DEA registration.
  • Insurance: Must maintain Malpractice Insurance ($1M/$3M) and provide a COI naming our Company as an additional insured.

Option 2: The Supported Professional Model (Turnkey Practice)Compensation & Benefits

  • Productivity-Based Pay: Competitive percentage of collections based on an insurance/fee-for-service model.
  • Zero Overhead: All clinical supplies, debridement tools, and mobile documentation technology are fully paid for and provided by the group.
  • Operational Support: We handle all facility scheduling, patient billing, and insurance credentialing.
  • Support Staff: We provide a medical assistant/scribe if needed to assist with resident positioning, wound measurement, and real-time charting.
  • Travel Stipend: Professional travel reimbursement/stipend provided for travel between facility locations.

Key Responsibilities

  • On-Site Clinical Rounds: Perform bedside wound evaluations and treatments for residents, focusing on chronic, non-healing wounds.
  • Patient Advocacy: Develop personalized healing plans, collaborating with facility nursing staff and the Interdisciplinary Team (IDT) to optimize nutrition and offloading.
  • Multispecialty Collaboration: Work alongside our on-site podiatry and medical teams to ensure a comprehensive approach to limb preservation and infection control.
  • Digital Documentation: Maintain accurate electronic records, including wound measurements and progress photos, within our mobile EHR platform.

Qualifications

  • Degree: MD/DO, NP, or PA from an accredited program with an active Missouri license. (Licensure in Arkansas and/or Kansas is a plus).
  • Certifications: WCC, CWS, or CWON certification is highly preferred.
  • Geriatric Interest: A genuine passion for working with seniors and managing complex co-morbidities (Diabetes, PVD, Dementia).
  • Adaptability: Comfortable working in varied long-term care environments and performing bedside procedures.

Pay: $150,000.00 – $350,000.00 per year
