We are seeking a PA-C who is energized by face-to-face patient care, values collaboration, and enjoys seeing the long-term impact of their work. This role blends primary care and wound management while allowing you to become a trusted medical presence within the communities you serve.
What You’ll Do.
- Provide on-site primary care, assessments, and wound care to senior patients.
- Manage chronic conditions with a proactive, preventative approach.
- Develop strong working relationships with nursing staff and care teams.
- Communicate clearly with families regarding care plans and progress.
- Maintain accurate, timely documentation to support continuity of care.
