Responsibilities
The Preventive/Comprehensive Cardiology practice utilizes a unique team approach to the primary and secondary prevention of heart diseases. In addition, it is a rapidly expanding area dedicated to providing care to patients with a broad spectrum of cardiology needs. The Nurse Practitioner/Physician Assistant in this role will be integral to managing a diverse patient population, utilizing a team-based approach to deliver high-quality, evidence-based care. This position offers the opportunity to work collaboratively with MD support while also exercising professional autonomy. Additionally, there will be numerous opportunities aligned with the Bold.Forward.Unbound strategies in the coming years, fostering growth and professional development within the practice.
Key Responsibilities:
- Conduct thorough assessments of patients with cardiovascular conditions.
- Educating and counseling individuals and families on cardiovascular disease prevention and wellness.
- Develop and implement individualized treatment plans with focus on lipid management, blood pressure management and weight loss pharmacologic therapy.
- Provide ongoing care and follow-up for patients with chronic heart conditions with emphasis on coronary disease and management of risk factors.
- Collaborate with multidisciplinary teams to coordinate comprehensive care.
- Diagnose and manage acute and chronic cardiac conditions.
Qualifications
Master’s or DNP prepared graduate of an accredited school of nursing and Nurse Practitioner. Current Minnesota RN license and certification as an FNP or ANP.
Or graduate of an accredited Physician Assistant program with current PA certification by NCCPA and licensure as a PA by the Minnesota board of Medical practice.
Must be eligible for DEA registration and billing privileges. BLS required.
Cardiology experience preferred but not required.
