Position Overview:
We are seeking a relationship-driven Physician Assistant to provide primary care, community rounding, chronic disease management, assessments, and collaborative care services throughout senior communities.
Candidates from family practice, urgent care, emergency medicine, hospital medicine, internal medicine, post-acute care, geriatrics, or traditional clinic settings are encouraged to apply.
Key Responsibilities:
- Perform patient assessments and follow-up visits.
- Manage acute and chronic medical conditions.
- Collaborate with nursing staff and interdisciplinary teams.
- Support transitions of care.
- Build strong patient and family relationships.
- Document care plans and maintain quality standards.
Qualifications:
- Graduate of accredited Physician Assistant program.
- Active Minnesota PA license required.
- Strong communication and relationship-building skills.
- Ability to work independently in a field-based environment.

Pay: $105,000.00 – $120,000.00 per year
