Hennepin County Health Care for the Homeless is seeking a part -time Nurse Practitioner or Physician Assistant who has an interest in and skill and experience working with individuals experiencing homelessness. In this role, you will provide health care services at local shelters and drop-in centers in downtown Minneapolis area. Chronic, preventative, and acute care services are provided on a walk-in basis to adults, youth, children, and infants. You’ll join a strong, diverse, multidisciplinary team of medical providers, registered nurses, social workers, licensed practical nurses and medical assistants. Salary will be based upon qualifications: Nurse Practitioner $76,961.87 – $161,203.28 Annually Physician Assistant $105,383.52 – $161,203.28 Annually Location and hours: This position is hybrid and will be on-site to provide health care throughout Minneapolis and may require travel between different locations, and remote as job duties required. Work hours are Monday – Friday, with rotating shifts being on call after hours, and may include one evening shift per week. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. This position is internally classified as a Physician Assistant. Click here to view the job classification specification. In the Nurse Practitioner position, you will:
- Provide medical consultation, care and education to youth, adults, and children.
- Perform initial and ongoing assessments of patient mental, physical, and psychosocial status, while doing triage for patients who require a higher level of care.
- Obtain and evaluate patient health and medical history and document in the EPIC Electronic Health Record (EHR).
- Perform physical examinations and conduct preventative screening procedures based on age and medical history; identify medical and health risks.
- Complete accurate and timely documentation of observations, interventions, referral, and follow-up; develop patient care plan.
- Provide health education, counseling, consultation, and advocacy based on needs identified in care plan.
- Provide supervision or lead work direction to professional and/or support staff, including other nurse practitioners.
In the Physician Assistant position, you will:

Need to have:
- For hire as a Nurse Practitioner:
- One of the following:
- Possession of a valid license as a Registered Nurse issued by the State of Minnesota and National Certification as a Nurse Practitioner (CNP).
- Possession of a valid license as an Advanced Practice Registered Nurse issued by the State of Minnesota.
- National Certification as a Family Nurse Practitioner (FNP)
- One of the following:
- For hire as a Physician Assistant:
- Graduation from an accredited physician assistant (PA) program.
- Certification or eligibility for certification from the State of Minnesota as a Physician Assistant. Must also complete the Delegation Agreement and Notice of Intent to Practice as required by the MN Board of Medical Practice.
Nice to have:
- Current CPR certification.
- Experience:
- Working as a Nurse Practitioner and/or Physician Assistant.
- Providing healthcare services to people experiencing homelessness.
- With healthcare disparities and inequities.
- Providing care to patients with substance use disorders (SUD).
- Working with Electronic Health Records (EHR).
- Ability to:
- Work independently and interdependently with professionals from other disciplines.
- Establish and maintain effective relationships with internal and external partners.
- Demonstrate strong leadership and written and verbal communication skills.
- Demonstrate sensitivity to the needs and issues of diverse individuals.
