Hennepin Healthcare has a flexible opportunity (0.5-1.0 FTE) for a Nurse Practitioner or Physician Assistant to work in our downtown Internal Medicine Clinic. This role has the possibility of also working at our Brooklyn Park Clinic or Hennepin County Jail. (Employees working at a level of 0.5 FTE and above are eligible for full medical/dental benefits, as well as several additional prorated benefits if less than full-time). Clinic hours are M – F during the day. Exact schedule TBD based on clinic need and provider preference.

This position is responsible for evaluation and treatment of patients with acute and chronic physical, mental health, and substance use conditions, as well as providing preventative care services and education to patients and families within scope of practice and in collaboration with the supervising/collaborating physician.

RESPONSIBILITIES

  • Provide direct primary care services to adult patients as part of the primary care interdisciplinary team in the ambulatory department
  • Perform initial and ongoing assessment of patients’ physical, mental health, and substance use conditions
  • Obtain and evaluate health and medical history, perform physical exam, conduct preventive screening procedures based on age and history, identify medical and health risks
  • Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up
  • Formulate appropriate differential diagnosis based on history, and physical examination
  • Identify needs of the individual, family or community and develop a plan of care and/or education plan
  • Order, conduct, and interpret diagnostic tests, therapeutic treatments; may prescribe, administer and dispense therapeutic devices and medications
  • Make appropriate referrals to other health professionals and community agencies
  • Reassess and modify the plan as necessary to achieve medical and health goals
  • Contribute to ongoing medical education teaching programs
  • Support new and existing programs which expand the organization’s role in the community
  • Participate in performance improvement activities
  • Adhere to productivity standards established by the department
  • Work closely with interdisciplinary teams to provide the best care for the population served
  • Respond timely to in-basket, forms, email, and paging communication regarding patient care
  • Demonstrate respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment
  • Perform other related duties but only after appropriate training

Minimum Qualifications

  • Master’s degree in Nursing or in Physician Assistant studies from an accredited program
  • Current license (or eligibility) to practice as an Advanced Practice Registered Nurse or Physician Assistant in the State of Minnesota
  • Certification in the area of specialization by an agency authorized to certify professional nurses in Advanced Practice or Physician Assistants
  • Knowledge of electronic medical documentation software/systems
  • Experience working with a diverse and complex patient population including patients who have substance use disorders

Preferred Qualifications

  • 2-3 years of NP or PA experience is preferred

SALARY Nurse Practitioner or Physician Assistant pay is based on years since licensure and specialty area. The grid below represents current pay rates (rates adjusted annually in April) for this position:
