Job Description: Contract Nurse Practitioner/Physician Assistant
Position OverviewOasis Child and Family Services is seeking a qualified Nurse Practitioner (NP) or Physician Assistant (PA) for a Contract Position to deliver medical care and support services at our Recuperative Care Facility program. This facility provides short-term stays (up to 21 days, or a maximum of 60 days as per Minnesota DHS guidelines) for 5-10 patients, primarily Minnesota Health Care Program (MHCP) members experiencing homelessness. The goal of the program is to help patients recover from physical illnesses in a stable environment and transition to permanent housing.
As a Nurse Practitioner or Physician Assistant, you will work collaboratively with Case Managers, Mental Health Professionals, and support staff to ensure comprehensive, patient-centered care.
About the Recuperative Care Program
Recuperative care offers a safe space for MHCP members who:
- Are unhoused and unable to recover from physical illnesses in shelters or on the streets.
- Have been discharged from hospitals but require continued medical care without meeting the criteria for hospitalization.
This program supports patients’ recovery through medical care and case management, facilitating their transition to permanent housing solutions, such as group homes or assisted living facilities.
Responsibilities
- Medical Care Delivery
- Perform patient assessments, including comprehensive physical exams and medical history reviews.
- Diagnose and treat acute and chronic health conditions within the scope of practice.
- Prescribe and manage medications, therapies, and other treatments as appropriate.
- Monitor patients’ progress and adjust care plans as necessary to ensure optimal recovery.
- Clinical Care Coordination
- Collaborate with interdisciplinary teams to create and implement individualized care plans.
- Facilitate smooth transitions between healthcare settings, including hospital discharge planning.
- Coordinate referrals to specialists, primary care, or community resources as needed.
- Regulatory Compliance
- Ensure compliance with Minnesota DHS and Recuperative Care Program guidelines.
- Maintain accurate, up-to-date medical records in accordance with state and federal regulations.
- Team Collaboration
- Work closely with Case Managers and Mental Health Professionals to provide holistic care.
- Provide guidance and medical expertise to support staff in delivering quality services.
- Engage in regular communication with team members to ensure seamless patient care.
- Patient Advocacy and Transition Support
- Support patients in accessing appropriate medical and social services.
- Collaborate with Case Managers to develop discharge plans, including transitioning to group homes or assisted living facilities.
- Advocate for patients’ needs to secure housing and other essential resources.
Required Credentials and Qualifications
- Licensure:
- Nurse Practitioner: Master’s or Doctorate in Nursing (MSN/DNP) with licensure in Minnesota and national certification.
- Physician Assistant: Graduate from an accredited PA program with licensure in Minnesota and NCCPA certification.
- Current DEA license and authority to prescribe medications.
- Experience providing care to vulnerable populations, particularly individuals experiencing homelessness or those with chronic medical conditions.
- Knowledge of Minnesota DHS guidelines and recuperative care models.
Preferred Qualifications
- Experience in transitional care, community health, or discharge planning.
- Familiarity with housing resources and social support systems in Minnesota.
- Ability to work in culturally diverse environments and deliver trauma-informed care.

- Hours: Flexible, based on patient care needs and admissions.

Compensation
