Job Title: Nurse Practitioner or Physician Assistant – General Cardiology
University of Minnesota Physicians is seeking a Certified Nurse Practitioner or a Certified Physician Assistant to join our General Cardiology practice in the Central Region (Minneapolis).
In This Role, You Will:
- provide and coordinate care for adults with cardiac diseases throughout the course of evaluation and treatment, including hospital admission, intervention, inpatient care, and discharge planning
- provide and coordinate continuity of care between the hospital floor setting, procedure room, and clinic. This includes clinic follow-up, providing information to referring physicians and institutions, and patient/family education
- share responsibility for assessment and management of patient care. write admission and daily notes, orders and direct discharge planning and follow up.
- work to improve the quality of care to decrease length of stay on the inpatient service.
- obtain patient histories, perform physical and psychological health assessments, provides patient and family education and counseling, insures continuity of health care services.
- show commitment to exemplary patient care.
- manage patient care through appropriate use of available resources.
- demonstrate ability to deliver care or service adjusting approaches to reflect developmental level of population served.
- utilize fully integrated Electronic Medical Record (EPIC).
Schedule:
- 1.0 FTE, alternating 10-hour days
- 1:6 weekend coverage
- Position will be a hybrid of outpatient clinic and inpatient coverage
- Primary site will be UMMC with possible coverage at Maple Grove or Fridley clinics
We’re Looking For a PA or NP Who Is:
- A graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program.
- Eligible or is certified through NCCPA, ANCC or AANP.
- Licensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or MN APRN license or eligible to acquire a license and prescriptive authority through MN Board of Nursing (NP’s).
- Able to meet threshold criteria/qualifications for credentialing and privileges.
