Full Job Description

We are looking for an experienced Nurse Practitioner or Physician Assistant to diagnose and treat health conditions, as well as promote good health practices. As an Aeris Provider, you’ll be a key member of our team with the goal of providing the best care possible to our patients, conducting physical examinations, interpreting lab tests, and overseeing patients’ health.

You’ll be both a health practitioner and an educator. Education is an important value that Aeris provides to patients, families, and facility staff. We value a team spirit and a positive attitude. Provider Responsibilities:

  • Examine patients and their medical records
  • Order and study diagnostic tests
  • Diagnose health conditions and illnesses
  • Propose treatments for chronic and infectious diseases
  • Prescribe medications
  • Maintain accurate records and schedules
  • Inform patients about their health conditions
  • Educate patients on diseases and prevention
  • Monitor patients’ overall health and wellbeing
  • Promote positive health practices
  • Advance professional knowledge and conduct research
  • Communicate well with Aeris Medical Group staff, facility staff, and the patient’s family

Nurse Practitioner Skills:

  • 3-5+ Years of proven experience as Nurse Practitioner or Physician Assistant in a skilled nursing facility or similar setting
  • Expertise in geriatric care, primary care, acute care and chronic care practices
  • Knowledge of health and patient care regulations in the State of Minnesota
  • Excellent communication skills
  • Strong ethics and high integrity
  • Team spirit with a can-do attitude
  • High level of Patience
  • A drive to continue gaining knowledge and clinical experience
  • Valid provider license

Job Type: Full-time

Pay: $110,000.00 – $140,000.00 per year
