Full Job Description
We are looking for an experienced Nurse Practitioner or Physician Assistant to diagnose and treat health conditions, as well as promote good health practices.
You’ll be both a health practitioner and an educator. Education is an important value that Aeris provides to patients, families, and facility staff. We value a team spirit and a positive attitude. Minimum Qualifications:
- Minimum of 1 year of experience as Nurse Practitioner or Physician Assistant in a skilled nursing facility, acute care, sub-acute care or similar setting
- Knowledge in geriatric care, primary care, acute care and chronic care practices
- Knowledge of health and patient care regulations in the State of Minnesota
- Valid provider license
Provider Responsibilities:
- Examine patients and their medical records
- Order and study diagnostic tests
- Diagnose health conditions and illnesses
- Propose treatments for chronic and infectious diseases
- Prescribe medications
- Maintain accurate records and schedules
- Inform patients about their health conditions
- Educate patients on diseases and prevention
- Monitor patients’ overall health and wellbeing
- Promote positive health practices
- Advance professional knowledge and conduct research
- Communicate well with Aeris Medical Group staff, facility staff, and the patient’s family
Job Types: Full-time, Part-time, PRN, Per diem
Pay: $65.00 – $80.00 per hour
