The Nurse Practitioner and or Physician Assistant functions independently in a primary care practice, providing primary health care services to clients throughout the life-span in an outpatient setting. This expanded role includes health promotion, education and diagnosis and treatment of common health deviations.

This is a sourcing requisition for multiple sites and specialties including: Family Medicine, Internal Medicine, OBGYN, Pediatrics, and Psychiatry.

Requirements:

  • Current nurse specialty certification by the Michigan State Board of Nursing.
  • Licensed to practice nursing in the state of Michigan.
  • Two years’ experience working in a clinic setting as an APP or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above.

Responsibilities:

  • Provide diagnosis, treatment, prescription, referral, consultation, and follow-up for clients in a primary care setting.
  • Participate in the development of standards of care, protocols, quality monitors and administrative functions as necessary so that clients receive safe, quality care and referrals are made appropriately.
  • Serve as a liaison with the medical community and establishes and maintains linkages with external agencies and organizations.
  • Attend regular internal and external departmental meetings to discuss progress in meeting clinical program an agency goals.
  • Communicate appropriately with professionals to facilitate CSHS and enhance professional interdisciplinary relationships.
  • Participate actively (in area of expertise) in community programs to project a positive image of Cherry Street Services Inc. and support health education in the community.
  • Participate in after hour call system and hospital rounding as negotiated.
  • Understands and adheres to CSHS’s compliance standards as they appear in CSHS’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
  • Keep abreast of all pertinent federal, state and CSHS’s regulations, laws, and policies as they presently exist and as they change or are modified.
  • Ensure that the staff are trained and evaluated on their knowledge of and adherence to compliance policies and procedures specific to their jobs.
  • Participate in after hour call system (approx. 4-5 weeks per year. Rotated amongst all Cherry Health provider, calls triaged)
  • Insure appropriate referrals which encourages continuity of care.
  • Insure appropriate physician intervention.
  • Other duties as assigned.
