About the role:
As Accompany Health grows, the Behavioral Health Advanced Practice Clinician (BH APC) will be an important clinician in the BH team. The BH APC will work side-by-side with our Director of Integrated Behavioral Health to see patients primarily virtually, with the potential for in-person visits, in the Detroit market as well as in future markets. The BH APC reports to our National Director of Integrated Behavioral Health, and has an important voice in the development and implementation of our BH programs across the organization.
This is a primarily clinical role and the BH APC will function as a key part of our Integrated Care Team, which also include physicians, primary care advanced practice clinicians, community health workers, patient experience navigators, social workers, Registered Nurses, psychiatrists, and pharmacists. Together this team is responsible for providing and coordinating holistic, patient-centered care for an intimate panel of patients with complex medical, behavioral health, and social needs in an in-home and community-based model.
As an Accompany Health BH APC, you will care for patients primarily virtually via video, telephone, or text, but also in the home and community as needed as we expand our care model. You will primarily be seeing patients in our Bridge Care program, which may involve clinical diagnosis, treatment including medications, and coordination of care for patients with severe mental illness (SMI), including substance use disorders. You will also help establish our Advanced Behavioral Health pathway(s), as we discover the right way to support our patients with SMI who are either linked to community providers or have had difficulty following in the community. You will participate in our weekly Behavioral Health Huddle and our Behavioral Health Case Conferences, and may be asked to participate in our Multidisciplinary Case Conferences with the entire care team. We anticipate that 80% of this role will be clinically seeing patients and 20% will be administrative in non-direct patient care roles.

Responsibilities will include:

  • Clinical
  • Provide psychiatric diagnostic evaluations for patients referred to BH services.
  • Prescribe medications for depression, anxiety, PTSD, bipolar disorder, schizophrenia, substance use disorders, and other DSM5 diagnoses. This includes prescribing controlled substances in accordance with a Collaborative Practice Agreement.
  • Design and participate in patient treatment plans, particularly for complex patients
  • Order appropriate laboratory testing and other monitoring for patients.
  • Respond to clinical queries to keep documentation compliant.
  • Collaborate with Psychiatrists, Behavioral Health Clinicians, Primary Care APCs and Physicians, Care Team Managers, Community Health Workers, Patient Experience Navigators, and other team members to coordinate patient care.
  • Supporting patients in transitions of care from facilities for admissions related to behavioral health diagnoses, including but not limited to: coordinating on discharge plans with inpatient care management, performing assessments to identify risk factors for readmissions, ensuring close follow-up and coordinating services.
  • Assisting the clinical team in urgent triage for behavioral health related issues
  • Ensuring timely and appropriate documentation.
  • Collaborate with guardians, relatives, and significant others on client’s conditions and treatment when clinically appropriate.
  • Ability to complete the majority of visits in telehealth (video) or phone visits, as well as potential for some in-home care for selected patients.
  • Administrative
  • Providing feedback and behavioral health programs policies and processes to improve the quality of care.
  • Provide feedback, input, and testing for future product builds.
  • Assist in wellbeing and diversity, equity, and inclusion initiatives.
  • Co-develop behavioral health programming for training of BHCs and Accompany Health employees on pertinent behavioral health topics.
  • Participating in multi-disciplinary case conferences and clinical rounds to ensure holistic and appropriate care for patients.
  • Providing formal and informal coaching and education to other clinical and non-clinical team members about supporting patients with behavioral health issues.
  • Working with the National Director of Integrated Behavioral Health in other measures not mentioned above.
  • Roles and responsibilities may evolve as our care model develops.

Desired skills and experience:

  • Required
  • Certification by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric Mental Health Nurse Practitioner (PMHNP) or Physician Assistant with Psychiatry CAQ
  • Active DEA Controlled Substance Registration or eligible for application
  • Active Unrestricted license to practice as an advanced practice registered nurse (APRN) or Physician Assistant in the state of Michigan
  • Active controlled substance license in Michigan
  • Certified in Basic Life Support for Healthcare Providers
  • Comfort and experience in working within an electronic medical record
  • Valid unrestricted driver’s license and access to an insured vehicle for daily use
  • 2+ years of experience in assessing, provisional diagnosis, planning, and managing behavioral patient care as acquired through clinical experience
  • 5+ years of experience working in high risk populations with serious mental illness and substance use disorder
  • Expertise with DSM5 diagnostic criteria
  • Highly skilled in trauma-informed care and culturally sensitive care
  • Experience and comfort working within an interdisciplinary care team, and specifically closely collaborating with community health workers and primary care providers
  • Highly flexible and adaptable; enjoys learning new skills and contributing to ongoing learning and process improvement
  • Comfort with electronic medical record documentation and excited about how technology can support your work
  • Experience as an active participant in continuous quality improvement projects
  • Valid unrestricted driver’s license and access to an insured vehicle for daily use
  • Preferred
  • 2+ years experience in either community mental health setting or psychiatric clinic serving patients with severe mental illness
  • Experience in transitions of care management for patients discharging from behavioral health facilities or step-down units, including performing detailed medication reconciliation, patient education, and connection/navigation to appropriate services
  • Experience in value-based care organizations
  • Prior work experience as part of a multidisciplinary care team
