Physician Assistant (PA)/Nurse Practitioner(NP)
Skilled Nursing Facility / Nursing Home : No Overnight On-Call, Monday through Friday daytime hours, No weekends and No Holidays
Job Description:We are seeking compassionate and motivated Physician Assistants or Nurse Practitioners to provide high-quality medical care in a skilled nursing facility setting. This position offers an excellent work–life balance, with no overnight on-call, within a supportive and collaborative team environment. Providers will be assigned to an anchor facility while maintaining flexibility to assist at other affiliated facilities as needed for PTO coverage, census fluctuation and continuity of care
Responsibilities:
- Perform patient evaluations, assessments, and follow-up visits
- Develop and manage treatment plans in collaboration with the supervising physician
- Order and interpret diagnostic studies
- Maintain accurate and timely documentation
- Communicate and build relationship with nursing staff, families, and interdisciplinary teams
Position Highlights:

Qualifications:
- Active Physician Assistant license or Nurse Practitioner license (DEA, Controlled substance, etc.)
- Nursing home or geriatric experience preferred, not required
- Strong clinical judgment and communication skills
