Age Management of West Michigan is seeking a compassionate, clinically skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our team serving veterans residing at the Grand Rapids Veterans Home.

Position Responsibilities

As an NP or PA, you will provide comprehensive medical care to veteran residents while partnering with nursing, therapy, and other members of the interdisciplinary healthcare team.

Responsibilities include:

• Provide comprehensive primary and ongoing medical care to veteran residents.
• Perform physical examinations and comprehensive health assessments.
• Diagnose and treat acute and chronic medical conditions.
• Develop, implement, and monitor individualized treatment plans.
• Order, review, and interpret laboratory tests and diagnostic studies.
• Prescribe medications and monitor their effectiveness, safety, and potential side effects.
• Manage chronic medical conditions and appropriately evaluate and respond to acute changes in residents’ health.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
• Participate in resident care planning and care coordination.
• Communicate clearly, respectfully, and compassionately with residents, families, caregivers, and facility staff.
• Maintain accurate, timely, and thorough medical records and clinical documentation.
• Advocate for residents’ needs while delivering respectful, compassionate, and individualized care.

QualificationsRequired

• Current Michigan Nurse Practitioner or Physician Assistant license in good standing.
• Board certification as an NP or PA.
• Current DEA registration, as applicable, or the ability to obtain one.
• Strong clinical assessment, diagnostic, and decision-making skills.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Compassionate, dependable, and committed to providing exceptional patient care.

Preferred

• Experience in skilled nursing, long-term care, geriatrics, primary care, or a similar clinical setting.
• Experience caring for medically complex or older adult populations.
