Family Nurse Practitioner (FNP) or Physician Assistant (PA) – Full-Time

Chelmsford Family Practice is seeking a Full-Time Family Nurse Practitioner (FNP) or Physician Assistant (PA) with 2+ years of clinical experience to join our well-established, independent primary care practice. This is an excellent opportunity to work in a collaborative, patient-centered environment with strong clinical support and flexible scheduling.

- Team includes 3 physicians, 5 NPs, and 1 PA

- Affiliated with Lowell General Hospital (Tufts Medicine)
- Integrated care with Collaborative Care Model (CoCM) and Chronic Care Management (CCM) programs
Position Details
- Full-time, 4 days/week (Monday–Friday) with flexible scheduling options
- Dedicated nurse support for 1:1 collaboration
- Small-practice feel with opportunities to contribute to office-wide decisions
- Emphasis on consistent, high-quality, patient-centered care
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer basic life support and manage critical care situations
- Utilize Epic EHR for accurate documentation and patient tracking
- Collaborate with healthcare teams to provide optimal patient care
- Educate patients and families on health management and treatment options
- Stay updated on advancements in nursing practice and relevant medical technologies
Qualifications
- Current FNP-C or PA-C certification
- Proficient in patient assessment and care
- Experience with Epic
- Ability to work with diverse populations, including children and individuals with developmental disabilities
