Family Nurse Practitioner (FNP) or Physician Assistant (PA) – Full-Time

Chelmsford Family Practice | North Chelmsford, MAIndependent practice affiliated with Lowell General Hospital (Tufts Medicine)

Chelmsford Family Practice is seeking a Full-Time Family Nurse Practitioner (FNP) or Physician Assistant (PA) with 2+ years of clinical experience to join our well-established, independent primary care practice. This is an excellent opportunity to work in a collaborative, patient-centered environment with strong clinical support and flexible scheduling.

  • Team includes 3 physicians, 5 NPs, and 1 PA

  • Affiliated with Lowell General Hospital (Tufts Medicine)
  • Integrated care with Collaborative Care Model (CoCM) and Chronic Care Management (CCM) programs

Position Details

  • Full-time, 4 days/week (Monday–Friday) with flexible scheduling options
  • Dedicated nurse support for 1:1 collaboration
  • Small-practice feel with opportunities to contribute to office-wide decisions
  • Emphasis on consistent, high-quality, patient-centered care

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans
  • Administer basic life support and manage critical care situations
  • Utilize Epic EHR for accurate documentation and patient tracking
  • Collaborate with healthcare teams to provide optimal patient care
  • Educate patients and families on health management and treatment options
  • Stay updated on advancements in nursing practice and relevant medical technologies

Qualifications

  • Current FNP-C or PA-C certification
  • Proficient in patient assessment and care
  • Experience with Epic
  • Ability to work with diverse populations, including children and individuals with developmental disabilities
