Structural Heart Program rapid expansion in patient volume and procedures

Job Summary

The Advanced Practice Provider develops, implements, and evaluates a program of specialized cardiovascular health care. The APP promotes a collaborative relationship among various health care professionals toward continuity of patient care and efficient use of resources. The APP serves as a role model for professional practice in the management of cardiovascular patients. In accordance with the rules and regulations of the state of Massachusetts and the MGH governing Advanced Practice Providers (NP/PA) practice, and under the general supervision of the Director of Advanced Practice Providers and the Medical Director for Interventional Cardiology, the APP provides direct care to patients in a high-volume clinical service. S/he develops, implements, and evaluates a program of specialized health care for a select patient population with cardiovascular disease. S/he/they will be intimately involved in direct care and education of patients, and should have experience working with patients requiring diagnostic and/or invasive procedures performed in a cardiac catheterization lab. The APP promotes a collaborative relationship among various health care professionals toward continuity of patient care and efficient use of resources. The APP will be the primary clinical triage for the Heart Valve program including patients with heart failure and valvular heart disease undergoing specialty valve procedures including TAVR, Mitral Clip, tricuspid valve repair.. S/he will will work with the program RNs and coordinating intake and clinical triage of new referrals including interpretation of lab and testing data and recommend additional testing as needed. S/he/they will coordinate follow up testing or referrals, order diagnostic studies and interventional procedures, and will have independent clinics to perform pre and post procedure follow visits for disease and symptom management. S/he will serve as a relationship manager and liaison between the program and referring physicians ensuring easy access to the program, continuity of patient care, and efficient use of resources.

Qualifications

UNIVERSAL DUTIES AND RESPONSIBILITIES:

1. Provides direct care to a select patient population in collaboration with the physician.

Performs and records the patient history and physical Utilizes advanced knowledge to identify, prevent or solve complex and/or recurring patient care problems. Follows and manages patients (in collaboration with MDs) enhancing continuity of care. Performs medically delegated aspects of patient care according to practice guidelines. Collaborates with staff and physicians to facilitate a comprehensive plan of care and coordinate resources. Writes orders which are in accordance with current guidelines. Maintains and practices in accordance with practitioner guidelines per state and hospital regulations

Prescribe medications in accordance with guidelines developed with the supervising physician(s)

Scope of Prescriptive Practice

Prescribe Schedule II-VI medications in accordance with guidelines developed with supervising physician Maintain a current license from the Massachusetts Department of Public Health, Division of Food and

Drugs

Maintain a certificate for prescribing controlled substances from the Drug Enforcement Administration Monitors patient outcomes in accordance with defined standards of patient care.

Facilitates patient movement through the system and efficient use of resources through interdepartmental collaboration.

Serves as a resource to staff in area(s) of expertise. Participates in the development of formal and informal educational programs. Responsible for own professional growth and maintenance of required credentials.

Maintains the standards and educational requirements specific to the position and licensure.

Participates in MGH and Partners quality initiatives and/or hospital committees as assigned

Heart Valve PROGRAM SPECIFIC RESPONSIBILITIES:

Clinical Practice

Triage patients to appropriate therapeutic pathways and clinical research protocols Perform and/or oversee follow up clinical evaluations Provide individualized pre and post procedure care to patients in a independent clinic including patient teaching and family counseling Insure uniform and comprehensive clinical data collection Works collaboratively with the Program RN to ensure appropriate laboratory, radiologic, and diagnostic studies are ordered and offers clinical interpret of studies performed/ordered Obtain informed consent for specialized procedures Initiate consultations/referrals, and consistent and appropriate follow-up care in collaboration with referring and/or primary care physicians Runs an independent pre procedure and post procedure follow up clinic.

Education Master’s Degree Nursing required Can this role accept experience in lieu of a degree?   • Skilled in taking medical histories to assess medical condition and interpret findings.
  • Ability to maintain quality control standards.
  • Ability to react calmly and effectively in emergency situations.
  • Ability to interpret, adapt and apply guidelines and procedures.
  • Ability to communicate clearly and establish/maintain effective working relationships with patients, medical staff and the public.
