The MGH Abdominal Transplant Program seeks a qualified Advance Practice Provider (APP)( Nurse Practitioner/Physician Assistant) to join our large multidisciplinary program. The successful candidate will join a program dedicated to a mission of excellence in patient care, dedicating to sharing of knowledge and advancement of transplant science. The position reports to Supervising MD, Chief of Abdominal Transplant and administratively to the Sr. Administrative Director of the Transplant Center In accordance with the rules and regulations of Massachusetts governing Nurse Practitioner (NP) or Physician Assistant (PA) will be responsible for ensuring continuity of care of liver transplant recipients and will work collaboratively with members of the multidisciplinary team. The APP is responsible for providing comprehensive care, assessing, diagnosing, and treating patients in collaboration with physicians and other healthcare professionals. The APP acts as the central point of contact and maintains open lines of communication between surgeons, physicians, transplant coordinators, social work and pharmacists.
- The APP will perform in the role of Independent Licensed Provider (ILP) and be responsible to work both independently and in collaboration with other providers. This role is for outpatient care (post-transplant).
- Assess, plan, implement and collaborate on needs and care of patients.
- Post Transplant: Manifest a high level of expertise in clinical evaluation, diagnosis and management of complex medical/surgical patients with actual or potential health problems related to their transplant, health maintenance and psychosocial factors following transplant.
- Participates in monthly meeting LIFT clinic (long term individualized follow up after liver transplant).
- Participates in weekly educational conference.
Qualifications:
- Current Nurse Practitioner or Physician Assistant Licensure in Massachusetts.
- Current certification as a Nurse Practitioner or Physician Assistant.
- Master’s Degree required
- Minimum of 2 years relevant advanced practice experience preferred. Specifically Hepatology or GI if no transplant experience.
Shift
- Day shift 4,10hr shifts 7:30-5:30 on site 3 days remote 1 day. This role is for outpatient care (post-transplant)
Job Summary
Summary The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation. Yes Essential Functions Provides direct care, counseling, and teaching to a designated patient population in the ambulatory, inpatient, operative, and/or procedural setting
- Performs complete histories and physical examinations
- Orders, interprets, and evaluates appropriate laboratory and diagnostic tests
- Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings
- Consistently provides high quality and timely documentation including admission and progress notes, procedure notes, operative notes and discharge summaries
- Performs bedside procedures as are appropriate to the patient population
- Orders medications and writes prescriptions according to organizational and regulatory policies and procedures
Qualifications

- Skilled in taking medical histories to assess medical condition and interpret findings.
