Job Summary:
***NOW OFFERING A $4,000 SIGN ON BONUS***
This unique position offers the opportunity for either a nurse practitioner or a physician assistant to assume both administrative and clinical responsibilities for the daily operation of the Family Team. Responsibilities include direct patient care as well as planning, implementing, supervising, and evaluating the delivery of patient care within established regulatory guidelines and professional practice standards. This exciting role offers the opportunity to utilize both your clinical and managerial skills and knowledge, as well as working with the BHCHP Family Team to deliver patient centered and trauma informed, care to children and adults who are at risk for, or are experiencing, homelessness.

Hours: Full time; Monday- Friday 8:30 am- 5:00 pm

Responsibilities:
Clinical Responsibilities

  • Oversee and monitor clinical operations, including direct care and management activities.
  • Provide comprehensive, high quality primary, episodic, and/or recuperative care.
  • Elicit an appropriate history, perform physical examinations, order diagnostic studies, interpret results and initiate therapy in accordance with the Guidelines for Physician Assistants and Nurse Practitioners of Boston Health Care for the Homeless Program.
  • Refer patients as appropriate to medical specialists, hospital admission, recuperative care and detox facilities, providing appropriate information for continuity of care.
  • Participate in collaborative relationship with BHCHP physicians according to the Regulations of the Commonwealth of Massachusetts.
  • Provide education to patients on health promotion and disease treatment and prevention.
  • Work as part of multi-disciplinary team in caring for patients.
  • Working with the care team, provide oversight of services offered at the clinic (primary care with addiction services, pediatric and adult nursing visits, phlebotomy).
  • Serve as the Vaccine Manager for the BHCHP Family Team.
  • Develop policies and procedures for clinical operations in conjunction with the Family Team Directors and BHCHP Leadership.
  • Develop and sustain an environment that supports excellence in clinical practice through planning and implementing programs that contribute to positive patient outcome and quality measures.
  • Participation in BHCHP committees: Clinical Operations, Management, Quality & Efficiency.
  • Provide rotating on-call coverage

Administrative Responsibilities

  • Interview, hire, supervise and evaluate staff according to established personnel policies and procedures of BHCHP
  • Establish and maintenance of the working collaborative relationship with the Horizons for Children executive administration and staff.
  • Provide oversight to management of clinical and staff schedules.
  • With a focus on equity, serve as a resource/mentor to staff regarding professional development, participation in BHCHP equity and diversity trainings.
  • Ownership of clinic regulatory processes to meet or exceed federal, state, and local regulatory standards.

Supervisory Responsibility:

  • Supervise the Registered Nurses and Medical Assistants

Qualifications:

  • Licensed by the Commonwealth of Massachusetts
  • Registered or eligible to be registered with the Massachusetts Department of Public Health and the Federal Drug Enforcement Agency
  • Willingness to obtain X waiver for prescribing Buprenorphine
  • Experience in working with underserved populations, mentally ill, homeless and/or persons with substance use disorders preferred.
  • Proficiency in Spanish preferred
  • Pediatric care experience required

Compensation and Benefits:
***NOW OFFERING A $4,000 SIGN ON BONUS***

  • The compensation starts at $96,000.00 and increases based on years of experience.
  • BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount. In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred dollars added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
