General Summary
The Advanced Practice Provider (NP/PA) Lead is a licensed provider. The APP is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation. The APP Lead will perform additional leadership duties on the team/department through indirect management of other Advanced Practice Providers and/or Allied Health Professionals.
This is a fully-remote position within the After Hours Call Program (AHCP) and the APP Lead is expected to work a designated shift from 5PM until 8AM on weekdays and 24/7 on weekends. The AHCP aims to provide virtual visits, telephonic support and clinical guidance to our patients when our primary care practices aren’t open (evenings, nights, weekends and holidays). We are seeking experienced, autonomous APPs to field adult and pediatric calls and conduct virtual visits within a clinically appropriate scope. All of our clinicians will have access to Schmitt-Thompson triage guidelines/protocols as well as comprehensive patient information via our fully-integrated EMR System, Epic.
Each APP Lead will be responsible for supervision, scheduling, training, chart review, mentoring and oversight of critical values and in basket during shift work.
Duties & Responsibilities
- Provides direct telephonic and virtual care, counseling, and teaching to a designated patient population in the ambulatory setting serving over 1,000 Primary Care providers
- Performs complete histories and virtual examinations
- Orders, interprets, and evaluates appropriate laboratory and diagnostic tests
- Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings
- Orders medications and writes prescriptions according to organizational and regulatory policies and procedures
- APP Leads will also perform additional duties, including but not limited to: indirectly manages other Advanced Practice Providers and/or Allied Health Professionals; provides support in staffing/scheduling as needed; becomes involved in daily clinical problem solving, on-boarding, and metric tracking; provides input on staff appraisals
- Master’s Degree in Nursing or Physician Assistant Studies required
- 4-6 years of experience as a Nurse Practitioner or Physician Assistant required
- Certified Massachusetts & New Hampshire Nurse Practitioner or Physician Assistant required upon hire
