Full Job Description

Overview:

The Physician Assistant / Nurse Practitioner Graduate Advanced Practice Provider (GAPP) works with both Inpatient and Outpatient physicians in a challenging, but highly rewarding, academic community-hospital setting. Physician Assistant / Nurse Practitioner Graduate Advanced Practice Practitioners collaborate with attendings, resident physicians, and students from allied schools within a team-based approach in Inpatient Medicine, Emergency Medicine, and Surgery rotations. The Physician Assistant / Nurse Practitioner Graduate Advanced Practice Practitioner will also work directly with physicians in high volume office settings during Outpatient rotations. The Physician Assistant / Nurse Practitioner Graduate Advanced Practice Practitioner will also have the opportunity to engage in Quality and Patient Safety Department projects and initiatives. Patient care will remain within the scope of the Maryland Board of Physicians Quality Assurance practice under the supervision of physicians for Physician Assistants. Patient care will remain within the scope of the Maryland Board Nursing for Nurse Practitioners.

Responsibilities:

  • The Physician Assistant / Nurse Practitioner GAPP will be assigned to clinical rotations in both inpatient and outpatient settings.
  • The Physician Assistant / Nurse Practitioner GAPP must participate in a structured educational program.
  • The Physician Assistant / Nurse Practitioner GAPP is expected to perform the following during inpatient rotations:
    • Perform initial history & physical examinations and initiate admission orders.
    • See, examine, evaluate assigned patients daily, discuss with attending provider and document care in a daily progress note.
    • Assess acute patient problems.
    • Initiate emergency or urgent treatment.
    • Communicate significant changes in patient condition to the patient’s attending provider.
    • Order and evaluate pertinent radiographic data on initial read only.
    • Meet with Medical Teams at the beginning of shift to identify and review patients ready to be discharged. Discuss issues related to discharges with appropriate resident or attending.
    • Meet with Case Management to proactively anticipate discharge dates.
    • Order, interpret, follow-up on pertinent laboratory data.
    • Perform final patient visit.
    • Assist in discharge planning and scheduling of follow-up visits and discuss same with patients.
    • Initiate referrals to Social Services.
    • Facilitate follow-up Home Health care needs
    • Complete assigned discharge summaries on patients in a timely fashion to include history of present illness, medical history, medications, allergies, social history, family history, physical examination, laboratory and x-ray data, hospital course (including rehabilitation plans, discharge medications, and discharge diagnosis).
    • Provide patient teaching and counseling regarding discharge treatment plan, medications, diagnostic tests, follow-up appointments, etc.
    • Discuss problems in patient discharges with the resident or attending provider/specialist
    • Prepare daily, written signout on patients and provide verbal signout to primary team
    • Exhibit flexibility in meeting the needs of the hospital
    • Other duties as assigned
  • The Physician Assistant / Nurse Practitioner GAPP is expected to perform the following during outpatient rotations:
    • See, examine, and evaluate assigned patients, discuss with attending provider, and document care in the patient chart
    • Assess acute patient problems.
    • Initiate emergency or urgent treatment.
    • Communicate significant changes in patient condition to the attending provider
    • Order and evaluate pertinent diagnostic tests and discuss with attending provider.
    • Order, interpret, follow-up on pertinent laboratory data.
    • Consult with other healthcare professionals
    • Provide patient and family education and counseling regarding all aspects of care
    • Initiate appropriate referrals to other providers along with collaborating with other multidisciplinary team members including but not limited to case management, social work, and home health services.
    • Exhibit flexibility in meeting all needs of the practice
    • Other duties as assigned
  • The Physician Assistant / Nurse Practitioner GAPP is expected to perform additional duties as assigned by the Department of Quality and Patient Safety

Qualifications:

  • Must have completed a certified Physician Assistant or Nurse Practitioner program.
  • License Requirements: Maryland PA License, Maryland NP Certification
  • Certification Requirements: NCCPA, ANCC/AAN
  • Must be computer literate or learn within 6 months.
  • Must be able to read, write, and speak the English language in an understandable manner.
  • Must be able to work efficiently and possess the ability to make decisions when circumstances warrant such action.
  • Must possess the ability to effectively communicate and deal tactfully with other personnel, visitors, patients, physicians, customers, and the general public.
  • Must be flexible, have personal integrity, and the ability to function effectively as a team with other employees.
  • Must be able to follow written and oral instructions.
