Full Job Description

The Division of Endocrinology, Diabetes, and Nutrition is currently recruiting a Physician Assist to be a part of the inpatient Glucose Management Team. The Glucose Management Team is comprised of a Medical Director, Physician Assistants, Certified Diabetes Nurse Educator, Pharmacist, and Social Worker. The team will manage inpatient glucose levels, provide education, assist in social needs and transitioning patients to the ambulatory care setting at discharge. As many as 1/3 hospitalized patients have hyperglycemia or underlying diabetes. This program aims to provide value-based care and improve the long-term health outcomes of these patients.

We are looking for energetic, team-oriented individuals interested in improving health outcomes, health literacy, and social barriers in an inner-city population.

Under physician supervision, responsible for diagnosing and treating hospitalized patients with diabetes in collaboration with physician colleagues. Provides continuity of patient care by facilitating and coordinating communication between the health care team, patient and patient’s family. Coordinates care to ensure quality, cost effective care; appropriate length of stay; and patient safety. Serves as link integrating relevant research and best practices. Performs professional duties and technical procedures in diabetes management such as, medication management, insulin pump and continuous glucose monitoring management, and diabetes education. Makes initial diagnosis and order appropriate tests and treatment.

Department Essential functions:

  • Perform initial evaluation and physical exam for patients with new onset or existing diagnosis of diabetes
  • Provide consultation services to various inpatient primary teams regarding inpatient glucose management plans during patient’s hospital stay
  • Provide appropriate outpatient treatment recommendations to ensure optimal glycemic control
  • Participate in daily multidisciplinary rounds with endocrinology attendings and discuss patient care
  • Provide education to patients and their families about their diagnosis
  • Collaborate with a multidisciplinary team to ensure a smooth transition to outpatient care
  • Attend weekly endocrinology grand rounds and core conferences

EDUCATION and/or EXPERIENCE

  • Graduation from an accredited physician assistant program and certified by National Commission on Certification of Physician Assistants (NCCPA) as a PA-C. Registered and currently recorded with the Maryland Board of Allied Health Providers in the State of Maryland. Certified in area of specialty. DEA and CDS eligible.
  • Current Basic Cardiac Life Support Certification is required
  • Ability to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s requirements relative to his or her age specific needs, and to provide the care needed as described in the unit/area/department policies and procedures
  • Highly effective verbal and written communication skills are required to interact with patient families, departmental units, medical and nursing staff on all essential matters. Demonstrated/documented effective interpersonal skills.
