Job Description

The Advanced Heart Failure & Cardiac Transplant Program is seeking an experienced and motivated APP to join our multidisciplinary team. The APP will provide specialized care for heart transplant candidates and recipients across inpatient and outpatient settings. Key Responsibilities

  • Manage pre and post heart transplant patients, including assessments, diagnostic review, and treatment planning.
  • Provide daily inpatient care, including hemodynamic management, immunosuppression oversight, and coordination of multidisciplinary rounds.
  • Conduct outpatient follow ups, adjust therapy plans, review labs/imaging, and support long term transplant outcomes.
  • Monitor patients closely for transplant related complications including rejection, infection, malignancies.
  • Management of chronic medical illnesses common among the transplant recipient (diabetes, HTN, CKD)
  • Educate patients/families on transplant processes, medications, and complication awareness.
  • Collaborate with interdisciplinary team including but not limited to surgeons, cardiologists, pharmacists, transplant coordinators, and social workers.
  • Participate in quality initiatives, regulatory compliance, and program development.

Qualifications

Qualifications

  • Acute Care Nurse Practitoner or Physician Assistant with active state licensure and national certification.
  • BLS/ACLS required; DEA as applicable.
  • Experience in cardiology, cardiac ICU, advanced heart failure, transplant, or critical care strongly preferred.
  • Strong clinical judgment, communication skills, and ability to work in a fast paced, multidisciplinary environment.

Schedule & Environment

  • Mix of inpatient and outpatient clinical duties.
  • May include call or weekend rotation based on program needs.

Additional Information

Pay Range: $54.34-$77.08
