Full Job Description
POSITION SUMMARY
Provide primary care services to adult patients with acute and chronic medical conditions to include performance of physical examinations, formulating assessment and treatment plans, utilizing diagnostic tests appropriately and counseling patients in collaboration with a primary care physician.
MINIMUM QUALIFICATIONS
- Bilingual with fluency in Vietnamese or Spanish preferred.
- Master’s degree in nursing with a specialization in Adult Care required.
- At least one year of full-time experience as a clinical nurse practitioner preferred.
- Possess prescriptive authority.
- Certified by the State Board of Medical Examiners.
- Board certified by, NCCPA, NBDPA or other certifying board required.
- Proficiency in physical assessment required.
- Ability to use various standard computer applications, including word processing
- applications, required.
- Excellent oral and written communication skills required.
- Ability to work in a culturally diverse, patient-centered, holistic atmosphere required.
- Flexibility to work in multiple clinical settings strongly preferred.
KEY RESPONSIBILITIES AND PERFORMANCE STANDARDS
- Patient Assessments
- Provide health care services to patients who walk into NOELA Community Health Center (NOELA CHC) or its affiliated healthcare facilities medical seeking care;
- Conduct comprehensive medical evaluations of patients, developing plans of care, and initiating and managing therapeutic interventions independently or in consultation with NOELA CHC physicians as indicated;
- Order or perform screening and diagnostic tests as indicated while being able to recognize abnormal findings and subsequently develop appropriate follow up plans;
- Provide appropriate patient education as required;
- Contribute to health education of groups of patients when needed;
- Conduct periodic nurse case management as needed;
- Triage patients for acuity of illness in clinic and via telephone;
- Communicate clearly and concisely with co-workers all relevant information necessary for quality patient care;
- Report all unusual conditions or patient reactions to the staff physician;
- Complete documentation of patient encounters in a timely manner.
- Quality Improvement Initiatives
- Assist with the implementation of clinic-wide continuous quality, improvement programs in collaboration with NOELA CHC physicians and other team members
- Assist with the identification of community resources available to help clients and their families and guidance to them for their use.
- Contribute to the development of new and/or improved community resources and patterns of health care deliver.
- Additional tasks
- Cross trains with other support staff to assist as needed as relating to patient care.
- Exhibits a willingness to learn new tasks.
- Asks questions before beginning an unfamiliar assignment.
- Participate in continuing medical education programs
