In collaboration with physicians and the interprofessional team, the VAD coordinator will be responsible for advanced practice clinical care including planning, evaluation, coordination, and implementation of care for patients requiring mechanical cardiac assistance and their families. Coordinates and provides education to VAD patients and families. Coordinates inpatient and outpatient clinical management of the VAD population including complex drug regimens and monitoring of patients’ clinical course and stability. Leadership for other VAD coordinators and engineers. Participates in “on call” responsibilities with additional VAD coordinators when hired and cardiac transplant coordinators. Assists with development of protocols, program certification, outcome analysis, and performance improvement.

GENERAL DUTIES

  • Plan, direct, implement and evaluate the complex management of clinical services for ventricular assist device (VAD) candidates and patients consistent with clinical expertise and position responsibilities.

  • Assume primary responsibility for VAD recipients and caregiver education, using age specific skills and competencies, based on learning needs, chronological maturation, emotional status, and cognitive ability.

  • Educate and serve as a resource to other departments to increase VAD awareness and competency throughout the organization, using VAD practice guidelines/policies/protocols as a guide.

  • Participate in coordination and implementation of hospital services for VAD recipients to include case management review and discharge planning.

  • Use age specific skills and competencies to assess and implement appropriate plan of care for VAD candidate within scope of practice, including ordering testing and prescribing medications and therapies, evaluation of patient responses/status, and collaboration with the HF and transplant cardiologists as needed.

  • Assume on-call duties, including organ procurement coordination and hotline calls.

  • Assist with development of clinical protocols, research-based protocols, practice guidelines, and implementation for patient care.

  • Coordinate VAD program, credentialing, and ongoing maintenance of standards.

  • Coordinate documentation, analysis/evaluation, and presentation of outcomes of VAD program.

  • Develop and maintain a variety of internal and external relationships to accomplish program objectives, including but not limited to referring physicians, transplant surgeons and cardiologists, consultants, immunologists, financial coordinator, social worker, dietician, psychiatrist, leaders, nurses, therapists, EMS, biomed engineers, APPs, faculty, Louisiana Organ Procurement Agency, industry reps, acute and post-acute SNF/rehab facilities, etc.

  • Participate in HF and/or VAD professional organizations. Obtain/maintain specialty certification in CV/HF nursing.

  • Works closely with Emergency Medical Services to assure optimal continuity of care, from pre-hospital to hospital and post-discharge when applicable.

  • Participates in and helps to coordinate community outreach activities.

  • Other related responsibilities as indicated.

EXPERIENCE QUALIFICATIONS

  • Preferred: 3-5 years of cardiac nursing experience, including HF, VAD care

EDUCATION QUALIFICATIONS

  • Required: Masters or Doctorate Degree as APRN or PA.

LICENSES AND CERTIFICATIONS

  • Required: Current LA Nurse Practitioner or PA license with prescriptive authority, ACLS, BLS.

  • Preferred: Board certification, such as Cardiac/Vascular Nursing (ANCC), CHFN (AAHFN), CSC (AACN), VAD-C (ICCAC), HF-Cert (HFSA), or CCKE (ACC)

SKILLS AND ABILITIES

  • Good listening and compassionate communication with patients and caregivers. Excellent communication skills (written and verbal), collaboration, and teamwork skills with other clinicians. Experience/interest in helping to lead teams, coach and develop others, present data and ideas. Enjoys performance Improvement, evidence-based practice, and clinical guidelines. Strong organization and computer skills, including word, excel, and powerpoint. Eager to learn and participate in professional development opportunities. Strong ability to manage time, work independently, delegate, and multitask in fast-paced environment. Analytical and problem solving skills. Flexibility to adapt to new equipment, procedures, processes as they evolve in the cardiovascular service line.
