JOB PURPOSE OR MISSION: Assists the physician in coordinating care appropriate for the needs for the age population served, as defined in the department’s scope of service and under the direction/supervision of the physician.
PERFORMANCE CRITERIA

JOB FUNCTIONS
ESSENTIAL JOB FUNCTIONS include, but are not limited to:
1. Collects data essential to assessment and indicates deviations from normal to the physician.
PERFORMANCE STANDARDS:
- Performs medication reconciliation as part of the clinical assessment on all new admissions and then ongoing during the course of treatment.
- Involves the patient, significant others and health care providers as appropriate.
- Identifies and coordinates the need for further assessment by discipline.
- Documents patient care assessments in the medical record.
- Responsible for completion of all patient care assignments while maintaining quality assurance standards.
- Consistently demonstrates clinical competence in nursing procedures and patient care.
2. Performs patient care and education according to the standards and the needs of the patient.
PERFORMANCE STANDARDS:
- Facilitates patient groups as it relates to the clinical needs of the patient and as ordered by the psychiatrist. Such groups may include but are not limited to: activities of daily living, medication management, symptom management.
- Prints patient medication educational material from the intranet and provides to the patient.
- Answers patient questions as it relates to the educational material.
- Provides individualized patient education to patients.
- Assists physician during drug reconciliation day at the clinic for ongoing medication reconciliation.
- Calls in prescriptions to pharmacies as ordered per the physician.
- Collaborates with the interdisciplinary health care team to deliver patient care utilizing nursing practice standards.
- Maintains patient rights.
- Documents all patient/family education and ongoing assessments.
3. Performs all other duties as assigned.
PERFORMANCE STANDARDS:
- Demonstrates an ability to perform a variety of duties, and maintains flexibility as patient care needs change.
- Demonstrates a consistently well-organized approach to all assigned activities.
EXPERIENCE REQUIREMENTS
1 year psychiatric experience preferred
SPECIAL SKILL, LICENSE AND KNOWLEDGE REQUIREMENTS
Current Louisiana RN licensure
HIPAA REQUIREMENTS:
Maintains knowledge of and adherence to all applicable HIPAA regulations appropriate to Job Position including but not limited to: Medical records without limitation both paper and electronic, patient demographics, lab and radiology results, patient information related to surgery or appointment schedules, medical records related to quality/data, patient financial information, patient related complaints, information related to patient location, religious beliefs and/or public health records.
SAFETY REQUIREMENTS:
Maintains knowledge of and adherence to all applicable safety practices appropriate to Job Position including but not limited to: Incident reporting, handling of wastes, sharps and linen, PPE, exposure control plans, hand washing, environment of care, and receiving orders for patients, and patient identification.
