Job Summary

Premier Palliative Care is seeking a compassionate Nurse Practitioner or Physician Assistant to provide inpatient palliative care services. The provider will work collaboratively with physicians, hospital staff, patients, and families to manage symptoms, facilitate goals-of-care discussions, and improve quality of life for patients with serious or life-limiting illnesses.

Responsibilities

  • Perform inpatient palliative care consultations and follow-up visits.
  • Conduct patient assessments, physical examinations, and document findings in the electronic medical record.
  • Develop and manage individualized care plans focused on symptom management and quality of life.
  • Lead goals-of-care conversations, advance care planning, and end-of-life discussions.
  • Collaborate with hospitalists, specialists, nursing staff, case management, social work, and hospice teams.
  • Coordinate transitions of care and hospice referrals when appropriate.
  • Complete timely, accurate documentation and coding.

Qualifications

  • Current Nurse Practitioner or Physician Assistant license.
  • Current DEA registration.
  • Hospital medicine, palliative care, hospice, internal medicine, family medicine, or critical care experience preferred.
  • Excellent communication, clinical judgment, and organizational skills.
  • Ability to work independently within a collaborative interdisciplinary team.
