Full Job Description
We are a Federally Qualified Health Center serving the low income and underserved in our area. We are searching for a caring and professional NP or PA to serve as a primary care provider. Main duties of primary care visits, post hospital follow up visits, chronic disease management, preventative screenings. Also, assist with routine strep tests, UAs, urgent care for other patients as needed. Work as part of a treatment team with medical assistant, case manager and other office staff.
