About You

The NP | Physician’s Assistant will have the following qualifications

  • FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA
  • Current Advanced Practice license
  • 3+ years of clinical experience as a FNP/PA in the primary care or home care setting
  • Working knowledge of chronic care management and various medication options
  • Current DEA licensure / Prescriptive Authority post-hire
  • Experience in Family Medicine, Internal Medicine, Gerontology, Palliative Care or home care
  • Highly relationship oriented and ability to passionately serve the aging and elderly population with high levels of compassion, communication, and interpersonal skills.
  • Clean malpractice history.
  • Proficient in general medicine, neuro, cardiopulmonology, and musculoskeletal exams.
  • Comfortable with technology; highly proficient in logging and reporting
  • Ability to travel within the market to provide care to patients in their home
  • Positive attitude with a solution-oriented approach and a comfort working within ambiguity
  • A low ego and humility; an ability to gain trust through communication and doing what you sayyou will do
  • Accuracy on risk capture, high quality metrics and closure rates
  • High patient satisfaction and improved health status including reduced hospitalization and ER
  • Critical thinking to address patient needs, providing the appropriate resources and follow-up care to improve stability

About the Job

The NP | Physician’s Assistant will have the following responsibilities:

  • Interview patients and compile patients medical histories to assist in determination of health status.
  • Perform physical examinations of patients, noting normal and abnormal findings.
  • Obtain specimens from patients and request routine laboratory tests and procedures; perform diagnostic tests, order non-routine laboratory tests or procedures as required.
  • Analyze and evaluate patient history, physical examination findings and laboratory values, and makes preliminary diagnosis of patients for review by physician.
  • Develop care/treatment plan for patient according to established protocols and procedures; confer and consult with supervisory physician and other professionals before implementing treatment when needed.
  • Provide education and guidance to patients and family members in maintaining good health and disease prevention.
  • Attend regularly scheduled continuing education programs to ensure current knowledge of procedures, health care plans and protocols.
  • Facilitate communication between CINQCARE team and other medical services. Request consultations with other departments, as necessary. Act as liaison with other medical services (social work, rehabilitation medicine, etc.).
  • Maintain clinical documentation and response to patient and staff messaging in accordance with policy.
  • Comply with all HIPAA regulations and maintaining security of protected health information.
  • And, of course, all other duties as assigned.

The working environment and physical requirements of the job include:

This position requires both in-home and office-based work. The job requires frequent travel for home visits and travel to physician offices, hospitals, sub-acute facilities, community partners and non-home-based market offices in all types of weather conditions. In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.

In this position you will need an ability to travel frequently by car and/or public transportation, the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 50 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
