POSITION SUMMARY: The Advanced Practice Provider (APP) in our School-Based Clinics (SBC) is responsible for assessing, diagnosing, and treating students with acute and chronic health concerns. Meeting established standards of care and quality benchmarks, they play a vital role in developing, monitoring, and altering comprehensive health care treatment programs. This includes gathering detailed patient data, consulting with other health care professionals, as needed, and implementing care plans to improve health outcomes. The APP maintains emergency plans for schools, educates school staff, documents patient encounters, and ensures adherence to quality measures. An experienced APP may provide medical supervision to other school-based workers and serve as a preceptor for APP students.

Essential Duties:

1. Assess, diagnose, and treat patients with acute and chronic illness concerns.

2. Meet accepted standards of care practice and quality benchmarks.

3. Develop, monitor, and alter a comprehensive health care treatment program through:

a. Gathering comprehensive data through history taking, physical examination, and evaluation of laboratory diagnostic testing, as indicated.

b. Consulting as needed regarding significant abnormal findings with other health care professionals.

c. Providing and implementing a plan of care for improving health outcomes

4. Create a comprehensive emergency plan and train school staff for emergency tasks.

5. Maintain up-to-date physical and electronic versions of sub and emergency information for school at all times.

6. Provide education for school staff regarding the role of SBC Program and the role of the APP at the beginning of the school year and ongoing training as necessary.

7. Provide health education to patients, families, and school staff.

8. Attend, at a minimum, one back-to-school event to educate parents regarding Shalom and the SBC Program and to collect completed, signed consent forms.

9. Maintain appropriate documentation of patient encounters. Follow Shalom’s policy for chart locking deadlines.

10. Accurately document quality measures in the EMR as instructed by the QAQI team. Work to meet percentage of goal for each indicator set forth by Shalom.

11. Maintain clinic supplies while adhering to monthly budget.

12. Practice evidence-based medicine. Maintain and expand current knowledge base regarding patient care. Obtain adequate CME credit to maintain state license and board certification.

13. Complete Peer Chart Review as assigned by CNO or designee.

14. Submit required number of charts with prescriptions written to the collaborating physician or other designee on a monthly basis, for chart review, in accordance with the collaborative agreement.

15. May provide medical supervision to medical assistant, registered nurse (RN), licensed practical nurse (LPN), contracted agency nurses, or other SBC staff as assigned.

16. APPs with at least two years’ experience as an APP are expected to serve or be able to serve as a student preceptor for nurse practitioner (NP) and physician assistant (PA) students form schools contracted with Shalom.

Education & Qualification Requirements:

  • Current Indiana APRN OR PA license or eligibility to apply
  • Master of Science in Nursing from an accredited school of nursing OR Master of Physician Assistant Science
  • Board certification as a pediatric or family Nurse Practitioner or Physician Assistant
  • Minimum 1-3 years’ experience in direct patient care
  • Current BLS certification
  • Current CSR and DEA certification (if applicable)

KNOWLEDGE, SKILL, ABILITY AND WORK DIFFICULTY:

· Be able to develop and maintain effective interpersonal relations; maintain confidentiality of data, information, and activities.

· Have a working knowledge of Microsoft platforms (e.g., MS Word, Excel, PowerPoint, etc.) and EMR software (eCW, a plus).
