At CenterWell, you’ll not only provide hands-on care inside the exam room but also delve deeper into the social determinants of health, understanding how they impact patient outcomes. - Serve 12-15 patients per day, affording you the opportunity to focus more time on individualized patient care
- Thrive within a value-based care model, actively improving the quality of life for your patients
- Encounter a diverse caseload, spanning from preventative care to managing chronic conditions
- Dedicate your time to clinical care, including patient interaction and administrative tasks related to patient care
Your Accomplishments are Highly Valued:
- Hold an active, unrestricted NP/PA license in the appropriate state
- 3-5 years of experience as an NP or PA in geriatrics and/or primary care
- Maintain current certification as an NP or PA
- Possess or have the ability to obtain a current DEA license
- Embrace the role’s patient-facing nature and participate in Humana/Senior Bridge’s Tuberculosis (TB) screening program if selected
- Showcase your passion for contributing to an organization dedicated to enhancing patient experiences
Preferred Qualifications:
- Familiarity with the Value-Based Care model
- Previous experience in outpatient senior care
- Knowledge of HEDIS measures
