Physician Assistant or Family Nurse Practioner

Full Job Description

Premier Family Medicine is seeking a Physician Assistant or Family Nurse Practitioner for full-time employment (4-day work week) as a member of our healthcare team to provide a broad range of primary care services. Candidate will maintain a close working relationship with physicians to provide and direct high quality family medicine care service. Position requires clinical skills, the ability to keep current with policy changes and sensitivity to confidential matters.

PFM is located on the SE corner of Indianapolis near I65 and Emerson Avenue.

Essential Functions:

·Independently develops the plan of care utilizing advanced assessment, knowledge, skills, teaching, and critical thinking to optimize patient outcomes.

·Consults and collaborates with other members of the health care team as appropriate to enhance patient outcomes across the continuum of care.

·Counsels and educates patients, families, caregivers, health care team members in the management of health care needs.

·Participates in periodic evaluation of care provided.

·May provide direct treatment and management of health conditions via prescriptive authority or referral to other health care providers or community resources. Prescriptive authority, if applicable, directed by the sponsoring physician, as required in the Collaborative Agreement.

·Ensures compliance with regulatory requirements and practice policies.

Job Requirements:

·Appropriate Degrees for Physician Assistant or Nurse Practitioner and Board Certification

·Nurse Practitioner or Physician Assistant Licensure in the state of IN

·DEA Licensure and Prescriptive privileges (or prescriptive privilege eligible)

·Requires critical thinking skills, decisive judgment and the ability to work with minimal or no supervision. Must be able to work in a stressful environment

·Experience in Family Medicine preferred
