Full Job Description

POSITION SUMMARY:

The Physician Assistant provides primary care services to children and/or adults as part of the provider team at Crusader Community Health collaborating with licensed MD/DO physicians. The Physician Assistant will collaborate and consult when necessary with patients, families, Crusader support staff, other advanced practice providers, OBGYN physicians as well as internal and external consultants in order to provide the appropriate high level of care. This position reports to the Lead Advanced Practice Provider of the Adult and Family Medicine Department and subsequently the Chair of the Adult and Family Medicine Department.

EDUCATION REQUIREMENTS:

· Graduate (or soon to be) of United States accredited Physician Assistant Program

· Unrestricted Illinois Physician Assistant license, Illinois Controlled Substance License, and Federal DEA license

· Current CPR Certification

· Spanish fluency highly desired
