Summary

We are seeking motivated Nurse Practitioners (NPs) and Physician Assistants (PA’s) who are eager to explore and advance their careers in physical medicine and rehabilitation (physiatry). As a Nurse Practitioner and Physician Assistants (PA’s) at Jardin Health, you will work closely with our partner locations around Peoria to deliver exceptional physiatry evaluations and treatments. Extensive training will be provided by our experienced Physiatrist physician for you.

Responsibilities

  • Perform comprehensive Physiatry evaluations including medical history reviews, physical examinations, and diagnostic assessments.
  • Diagnose and treat acute and chronic musculoskeletal conditions under supervision.
  • Develop individualized treatment plans including therapeutic interventions, medications, injections, and rehabilitation strategies.
  • Collaborate closely with our Physiatrist physician and healthcare teams at partner locations to ensure coordinated patient care.
  • Educate patients and families regarding disease management, preventive strategies, pain management techniques, and lifestyle modifications.
  • Maintain accurate patient records adhering to regulatory standards.
  • Actively participate in ongoing training sessions provided by our Physiatrist to enhance clinical skills in physiatry.
  • Contribute to quality improvement initiatives within our practice.

Qualifications

  • Graduate from an accredited Nurse Practitioner program or a Physician Assistants (PA’s).
  • Active Illinois Nurse Practitioner license or Physician Assistants (PA’s) liscense.
  • Interest in physical medicine & rehabilitation (physiatry); prior experience beneficial but not required—we provide comprehensive training.
  • Strong interpersonal skills; ability to effectively communicate with patients, families, physicians, and healthcare teams.
  • Ability to manage multiple responsibilities in various partner locations around Peoria.
  • Commitment to delivering compassionate, patient-centered care.
