Job Description: Physician Assistant / Nurse Practitioner
Department: Primary Care | Status: Full-Time Hours may vary including evening and weekends. On-call shared with other providers (1:4) | Location: 6374 Lincoln Ave Chicago IL 60659
Reports To: Site Lead / Chief Medical Officer (CMO) | Compensation: Based on Experience
Duties and Responsibilities:
- Understanding and willingness to deliver care in a primary care setting.
- Guide and assist in all aspect of primary care medicine including: Preventative medical care, Psychosocial needs, Acute medical care, Diagnostics, Treatment, Consultation, and Patient education
- Function independently to perform: Age-appropriate history and physical patient exams, Engage in the above for acute and chronic illnesses, Recognize critical illness and notify the appropriate entities.
- Collaborate with supervising physician as needed.
- Prescribe appropriate pharmacologic and non-pharmacologic treatments.
- Implement interventions in attempts at restoring patient’s desired physiologic status.
- Monitor effectiveness of interventions
- Make efforts to correctly diagnose common and less common ailments.
- Investigates clinical problems and offers sound medical solutions.
- Communication: Communicates effectively with other healthcare team members, patient, family members and healthcare designees
- Procedures: Performs procedures utilizing medically approved approaches and equipment appropriate to the patient’s age and size
- Patient Education: Provides information and instructions at a level of understanding appropriate to the comprehension level of the patient and/or significant others.
- Actively participates in Quality Improvement activities.
Qualifications:
o Graduate of accredited school (PA, NP)
o Current license
o Current provider card in CPR
o Minimum 2 years’ experience in field (new graduates may apply)
o Basic computer skills
o Familiarity with or capable of learning electronic health records.
o Current DEA and Controlled Substance License when applicable
o Ability to respond to healthcare emergencies.
o Communication skills and ability to work in both superior and subordinate role.
- Nurse Practitioner (NP)
o Masters in Nursing
o Current license as RN and Nurse Practitioner in the State of IL (Family or Internal Med)o Certification as a Nurse Practitioner by a national certifying body as approved by the Illinois Department of Regulation (AANP, ANCC)
- Physician Assistant (PA)
o Master’s degree in area of expertise
Current license as a Physician Assistant in the State of Illinois.
o Certification as a Physician Assistant by a national certifying body as approved by the Illinois Department of Regulation.

Pay: $98,000.00 – $115,000.00 per year
