GENERAL SUMMARY:
We are seeking a skilled and compassionate Physician Assistant to join our team, focusing on both in-person patient care as well as telehealth and remote patient monitoring. In this dynamic role, you will have the opportunity to blend your clinical expertise with cutting-edge technology to provide comprehensive, patient-centered care. The ideal candidate will be passionate about leveraging digital tools to proactively manage patient health, improve outcomes, and enhance the patient experience.
DESCRIPTION:
Duties and Responsibilities:
- Conduct traditional Physician Assistant duties, including patient assessments, diagnosis, treatment planning, and medication management.
- Utilize telehealth platforms and remote monitoring devices to conduct virtual consultations and monitor patient health remotely.
- Review and analyze data collected through remote monitoring devices, identifying trends, abnormalities, and opportunities for intervention.
- Collaborate with physicians and interdisciplinary team members to develop and implement personalized care plans for patients, integrating both in-person and remote care modalities.
- Educate patients on the use of remote monitoring devices, medication adherence, lifestyle modifications, and self-management strategies to promote wellness and prevent complications.
- Communicate effectively with patients via telehealth platforms, providing ongoing support, answering questions, and addressing concerns.
- Document all patient interactions, assessments, and interventions accurately and comprehensively in electronic health records.
QUALIFICATIONS:
Required Knowledge, Skills and Abilities:
- Graduate of an accredited Physician Assistant program.
- Current Illinois licensure as a Physician Assistant.
- Certification by the National Commission on Certification of Physician Assistants (NCCPA).
- Experience or strong interest in telehealth, remote patient monitoring, and digital health technologies.
- Exceptional communication and interpersonal skills, with the ability to engage patients effectively in-person and remotely.
- Strong clinical assessment and critical thinking abilities, with adaptability to evolving patient needs and technology solutions.
- Proficiency in telehealth platforms, electronic health record (EHR) systems, and remote monitoring devices.
- Ability to work autonomously and collaboratively within a remote team environment.
- Commitment to providing patient-centered care, promoting health equity, and optimizing patient outcomes.
Preferred Qualifications
- Previous experience as a Physician Assistant in primary care, urgent care, or a related clinical setting.
- Knowledge of chronic disease management, preventive care strategies, and population health principles.
- Familiarity with remote monitoring devices, wearable health technologies, and data analytics tools.
- Bilingual proficiency.
