Role Overview:
We are seeking compassionate and dedicated Nurse Practitioners / Physician Assistants to join our team. The ideal candidate will provide high-quality primary and specialty care, with a focus on LGBTQ+ health. This role requires a commitment to patient-centered care, cultural competency, and a collaborative approach to healthcare delivery.
Key Responsibilities:
- Conducts comprehensive patient assessments, including health history, physical examinations, and diagnostic tests.
- Develops and implement individualized treatment plans based on patient needs and clinical guidelines.
- Provides preventive care, health education, and counseling to patients.
- Manages chronic conditions, including but not limited to, HIV/AIDS, diabetes, and hypertension.
- Collaborates with interdisciplinary team members, including physicians, nurses, social workers, and other healthcare professionals.
- Maintains accurate and timely documentation in electronic health records.
- Participates in quality improvement initiatives and continuing education programs.
Qualifications:
- Master’s or Doctorate degree in Nursing or Physician Assistant from an accredited program.
- Current Nurse Practitioner or Physician Assistant license and certification in the state of Illinois
- Prior experience in a clinical setting, preferably in primary care or LGBTQ+ health.
- Strong knowledge of LGBTQ+ health issues and a commitment to inclusive care.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work effectively in a fast-paced and dynamic environment.
Preferred Qualifications:
- Experience in community health settings.
- Knowledge of public health principles and practices.
- Bilingual in English and Spanish
