Full Job Description
We are seeking an experienced Nurse Practitioner or Physician Assistant to join a well-established, NP and Physician-led internal medicine practice providing in-home primary care and services within independent living settings across the Chicago area.
This role is ideal for a clinician who values autonomy, prefers to manage most patient needs in-house, and is comfortable delivering care in patients’ homes. The practice emphasizes continuity, relationship-based care, and thoughtful clinical decision-making rather than rigid protocols.

Schedule & Workload
Full-time, Monday–Friday
Typical hours: 8:00 AM – 5:00 PM (flexibility with start/stop times as long as work is completed)10–12 patients per dayDriving required within a ~60-mile radius, from South Chicago up to Zion, IL
Compensation & BenefitsSalary: $140,000 startingSign-on bonus: $2,000Employment type: W-2
