Full Job Description

We are seeking a dynamic and compassionate Physician Assistant (PA) or Nurse Practitioner (NP) to join our Family Practice team. This role offers a rewarding opportunity to provide high-quality, patient-centered care across a variety of settings, including assisted living facilities, nursing homes, and outpatient clinic coverage with a flexible schedule.

The ideal candidate is motivated to deliver comprehensive care for both acute and chronic conditions, perform thorough assessments, and collaborate closely with physicians and interdisciplinary healthcare teams to improve patient outcomes. This position is best suited for someone who thrives in a community-based, geriatric-focused practice and enjoys working independently while being supported by a strong clinical team.

Responsibilities

  • Perform comprehensive patient evaluations, including physical exams, medical histories, and assessment of acute and chronic conditions
  • Provide ongoing care for patients in assisted living facilities, nursing homes, and outpatient clinic settings
  • Develop, implement, and manage individualized treatment plans in collaboration with supervising physicians
  • Order and interpret diagnostic tests, labs, and imaging to guide clinical decision-making
  • Document patient encounters accurately and timely using EMR systems (e.g., eClinicalWorks, PointClickCare or similar)
  • Coordinate care with nursing staff, specialists, and facility teams to ensure continuity of care
  • Participate in discharge planning and transitions of care when applicable
  • Maintain compliance with all applicable healthcare regulations, documentation standards, and best practices

Experience & Qualifications

  • Current and active PA or NP license (required)
  • Prior experience in assisted living and/or nursing home settings strongly preferred
  • Experience in primary care or family medicine is highly desirable
  • Proficiency with EMR systems such as eClinicalWorks and PointClickCare preferred
  • Strong clinical assessment and diagnostic skills
  • Ability to work independently and manage a diverse patient panel across multiple settings
  • Excellent communication skills and a patient-centered approach to care
