We are seeking a dynamic and compassionate Physician Assistant (PA) or Nurse Practitioner (NP) to join our Family Practice team. This role offers a rewarding opportunity to provide high-quality, patient-centered care across a variety of settings, including assisted living facilities, nursing homes, and outpatient clinic coverage with a flexible schedule.

Responsibilities
- Perform comprehensive patient evaluations, including physical exams, medical histories, and assessment of acute and chronic conditions
- Provide ongoing care for patients in assisted living facilities, nursing homes, and outpatient clinic settings
- Develop, implement, and manage individualized treatment plans in collaboration with supervising physicians
- Order and interpret diagnostic tests, labs, and imaging to guide clinical decision-making
- Document patient encounters accurately and timely using EMR systems (e.g., eClinicalWorks, PointClickCare or similar)
- Coordinate care with nursing staff, specialists, and facility teams to ensure continuity of care
- Participate in discharge planning and transitions of care when applicable
- Maintain compliance with all applicable healthcare regulations, documentation standards, and best practices
Experience & Qualifications
- Current and active PA or NP license (required)
- Prior experience in assisted living and/or nursing home settings strongly preferred
- Experience in primary care or family medicine is highly desirable
- Proficiency with EMR systems such as eClinicalWorks and PointClickCare preferred
- Strong clinical assessment and diagnostic skills
- Ability to work independently and manage a diverse patient panel across multiple settings
- Excellent communication skills and a patient-centered approach to care
