Full Job Description

A Primary Care practice in Barrington is looking for a Licensed Nurse Practitioner or Physician Assistant in Family Medicine and Internal Medicine.

Job Summary:

Responsible for all aspects of patient care, including diagnosis, treatments, and consultations in hospital and outpatient clinical office.

Duties would include but not limited to:

·Examine, diagnose, and treat assigned acutely ill hospitalized patients. Manage all assigned patients through the continuum of hospital care. Follow assigned patients upon admission to the intensive care unit throughout their hospital stay. Coordinate post-acute care for all assigned patients.

  • Order treatment regimens and tests, and interpret results of tests such as laboratory and radiology
  • Prescribe medications as appropriate and as required
  • Conduct patient contact/rounding/care management that actively encourages and includes professional input from all members of the patient care team
  • Consult with specialty and primary care physicians where appropriate
  • Communicate with LG Healthcare care team, family members and others upon admission, when treatment plans change, and at discharge to maintain continuity and quality of care
  • Conduct discharge planning and assure timely discharge of patients
  • Assure accurate and timely completion of all clinical and professional records and documentation, so as comply with local, state and federal regulatory, licensure and accreditation requirements
  • Provide general and preventative care in our outpatient office. Conduct check-ups, treat illnesses, order lab tests, and prescribe medications with or without a nurse or MA’s support
  • Assist physicians in reviewing lab results of patients in our outpatient office.
  • Collaborate with other LG Healthcare employees and providers to create a positive and engaging work environment
  • Active participation in the development/review of office policies and procedures.
  • Flexible in schedule to accommodate changing needs of patients and business.

EDUCATION/EXPERIENCE REQUIREMENTS:

  • NP or PA with current license to practice medicine in the State of Illinois.
  • Active DEA and Illinois Control Substance License required.
  • No barriers to obtaining medical staff privileges at the assigned hospital.
  • Two years of post master degree experience in family medicine or internal medicine are required. TIME SPENT ON CLINICAL TRAINING for degree or license requirements IS EXCLUDED.

Job Types: Full-time, Part-time. “Round and go” model for the work in the hospital.
