Wound Care Provider Job Description
Job SummaryWe are a locally owned private practice specializing in mobile wound care. We provide comprehensive wound care services to our patients wherever they call home which could be their own private residence or in facilities of varying types. Our approach to wound care is centered around the individual, with a focus on healing, wellness, and patient education. We collaborate with our care partners in facilities and Home Health Agencies to provide direction and guidance, always focused on the goal of faster healing for our patients.
We are seeking a dedicated and compassionate Nurse Practitioner or Physician’s Assistant to join our mobile wound care practice in a flexible, part-time capacity.
The ideal candidate will provide high-quality patient care, perform assessments, and develop treatment plans for patients across various settings. This role requires strong clinical skills, the ability to work independently and collaboratively with our internal team members, as well as our care partners in the community, while being committed to improving patient outcomes.
Duties
- Traveling locally between patients is a requirement (House Calls).
- Provide wound care to patients including, but not limited to, wound assessment, proper dressing choice/application, as well as ultrasonic and sharp debridement.
- Create individual treatment plans for varying wound types.
- Monitor vital signs and interpret diagnostic tests.
- Collaborate with physicians and other healthcare professionals in developing patient care plans.
- Educate patients and their families on health management, disease prevention, and wellness strategies.
- Manage home care services for patients requiring ongoing support.
- Maintain accurate patient records and documentation in compliance with regulations.
Requirements
- Board Certified Nurse Practitioner or Physician Assistant with 1-2 years of experience required; extra consideration for wound care experience.
- Ability to create individual treatment plans for varying wound types.
- Ability to interpret and evaluate laboratory and other diagnostic studies.
- Experience in geriatrics is helpful but not required.
- Excellent communication skills, both verbal and written, with the ability to communicate in a positive and professional demeanor.
- Capable and willing to work independently while setting own schedule and appointments with a focus on reliability.
- Familiarity with electronic health records (EHR) systems is required. We use E Clinical Works.
- Driving is a requirement and you must have reliable transportation; valid driver’s license and current auto insurance..

Job Type: Part-time
Pay: $60.00 – $70.00 per hour
Expected hours: 10 – 30 per week
