Position Title: Wound Care Advanced Practice Provider (Nurse Practitioner, Physician Assistant)
Department: Specialty Medicine and Surgical Clinic
Supervisor’s Title: Executive Director of Physician Services and Clinic Administrator
I Position Summary
The purpose of the Advanced Practice Provider is to be responsible for diagnosing and assessing and treating various wounds. Develop personalized care plans, educate patients and families, and potentially utilize advanced therapies like sharp debridement of wounds or negative pressure wound therapy. They work across various healthcare settings to promote healing and prevent complications in patients with acute or chronic wounds.
II Principal Functions and Responsibilities:
- Evaluate wounds to determine their type, stage, and underlying causes, including pressure ulcers, venous stasis ulcer, diabetic foot wounds, and surgical wounds.
- Diagnose and implement individualized wound care treatment plans to optimize wound healing and prevent complications.
- Perform wound cleaning, sharp debridement and dressing changes ensuring proper infection control.
- Utilize advanced wound care techniques, such as negative pressure wound therapy (NPWT) and cellular – and/or tissue-based products.
- Prescribe or administer treatment, therapy, medications and other specialized medical care to treat or prevent patient’s condition.
- Accurate and timely document for medical necessity, as defined by Medical Staff Rules, patient information including medical history, clinical condition, medical interventions utilized for treatment and plan of care.
- Write and order diagnostic studies for laboratory, x-ray, and other special tests as allowed in the Delegation of Services.
- Provide guidance and applicable delegation to clinical staff including, but not limited, to RN’s, LPN’s, MA’s, CNA’s, Diagnostic Imaging Techs, and/or Laboratory Techs/Scientists.
- Interpret reported information in accordance with knowledge of the laboratory or diagnostic studies, provided such laboratory or diagnostic studies are related to and consistent with the practitioner’s scope of practice and experience.
- Effectively conduct procedures and services that are within the scope of service and in accordance with Valor Health and clinic policies.
- Collaborate with primary care providers and other members of the healthcare team to ensure coordinated wound care management, including inpatient wound consultations and ongoing outpatient follow-up.
- Serve as a resource and consultant for other healthcare professionals regarding wound care management.
- Stay up to date with the latest advancements in wound care and best practices.
- Maintain confidentiality in matters relating to patient and family.
- Effectively interact with patients/ families with a variety of developmental and socio-cultural backgrounds.
- Maintain a safe environment by recognizing, eliminating and/or reporting hazards.
- Actively participate in Risk, Compliance, Quality, Performance Improvement, Patient Safety, and Infection Prevention programs as deemed necessary by the organization.
- Complete all necessary paperwork, including documentation to support claim submittal, records requiring provider certification and sign off on charting in a timely manner.
- Assure that adequate patient health records are maintained and transferred as required and in a timely manner.
- Follow organization policy, Medical Staff Bylaws and Rules.
- Other duties as assigned.
III Position Qualifications/Requirement:
- Current, unrestricted, Physician Assistant licensure in Idaho or current, unrestricted Advanced Practice Professional Nursing licensure in Idaho.
- Wound care certification (e.g. Wound, Ostomy and Continence Nurse (OCN), Certified Wound Specialist (CWS) preferred OR required within 1 year of hire date.
- Strong understanding of wound physiology, pathophysiology and treatment modalities.
- Current Basic Life Support (BLS) Certification Card
- Current DEA certificate without restriction.
- PALS or NRP, ACLS, as required.
- Ability and willingness to demonstrate and maintain competency as required for job title and the unit/area(s) of assignment.

- Ability and willingness to work as part of a team-based model for care delivery.
