J. C. Lewis Primary Health Care Center is a federally qualified community health center which exists to provide patient-centered primary health care services to the medically underserved residents of the greater Savannah-Chatham County, Georgia community. The Health Center makes comprehensive primary care, integrated behavioral health care, oral health care, and inpatient respite care services available to all persons, including the homeless and low-income uninsured and underinsured, regardless of ability to pay. Position Summary:

The J.C. Lewis Primary Health Care Center is seeking to add a Full-time Family Nurse Practitioner or Physician Assistant to our existing interdisciplinary team. The Family Nurse Practitioner (FNP) or Physician Assistant functions as an active member of the integrated care team to provide comprehensive integrated primary health care to children and adults in an outpatient primary care setting. The Family Nurse Practitioner or Physician Assistant supports the teams’ efforts by providing comprehensive assessments, diagnostic and treatment services with regard to physical and behavioral health conditions.

Qualifications:

  • Registered Nurse currently licensed in the State of Georgia
  • Advanced Nurse Practitioner currently registered with the Georgia State Board of Nurse Examiners (BNE)
  • Masters degree in Nursing, with extensive knowledge in providing comprehensive medical care.
  • Licensed as a Physician Assistant in the State of Georgia
  • Ability to establish and maintain relationships with the medical community
  • Ability to establish and maintain professional relationship with patients
  • Excellent oral and written communication skills
  • Non-profit or public health experience (preferred)
  • Pediatrics experience (preferred)

Primary Responsibilities:

  • Provide clinical services with competence and according to guidelines
  • Promote healthy lifestyles through health promotion teaching and screening
  • Formulate diagnosis on basis of assessment
  • Initiate treatment according to approved protocols and within scope of practice
  • Make appropriate medical and behavioral health referrals
  • Utilize support of staff and collaborating providers
  • Perform medical services required of position, including immunizations, as well as providing education to the clients on disease prevention
  • Performing physical exams with special emphasis on the reproductive system, including breast examination, pelvic examination, cancer screening tests, diagnosis of sexually transmitted infections, and other types of more specialized procedures as may be indicated by policy.
  • Providing relevant health instruction to include pre and post natal care, gynecological treatments and procedures, family planning, nutrition, sexual counseling, and principles of health promotion and maintenance.
  • Ensure that all medical cases and needs are documented appropriately
