Carroll Total Healthcare (CTHC) is not a traditional clinic. We are a concierge-style, precision medicine practice focused on optimizing health span, not just managing disease. We are seeking a mission-aligned Physician Assistants or Nurse Practitioners to provide primary care and urgent care services to our dynamic, self-pay and insurance pay patient base.

Key Responsibilities
- Provide evidence-based primary care and same-day urgent visits for adults and families.
- Manage acute illnesses, chronic disease optimization, and preventive screenings.
- Participate in comprehensive intake assessments including functional medicine-style histories.
- Order and interpret advanced diagnostic panels (e.g., hormone, micronutrient, inflammatory, GI).
- Coordinate care alongside our wellness and regenerative teams (pellet therapy, peptides, IVs).
- Use our internal Four Pillars of Health (Sleep, Nutrition, Exercise, Hydration) as clinical touchpoints.
- Deliver care within a concierge model — minimal insurance billing; documentation is focused and minimal.
- Provide patient coaching and wellness planning.
- Occasionally participate in member-only events, panels, or educational workshops.
Required Qualifications
- Current and unrestricted PA or NP license in Georgia.
- 2+ years of experience in primary care, urgent care, or family medicine.
- Comfortable working autonomously within a concierge setting.
- Passion for root-cause medicine, prevention, and optimizing vitality.
- Strong skills in patient communication and long-term care planning.
Preferred Experience (Not Required)
- Experience with hormone therapy, peptides, or functional medicine.
- Familiarity with cash-pay or DPC (direct primary care) models.
- Exposure to integrative wellness, such as nutraceuticals, IV therapy, or weight optimization.

Job Types: Full-time, Part-time
Pay: $90,000.00 – $110,000.00 per year
