Full Job Description
JOB PURPOSE: To work with the primary PruittHealth Premier team and provide patients with primary medical care services within the standard of care for Internal Medicine patients. This includes treating chronic illnesses and injuries. Advising preventative healthcare, performing physical exams, taking medical histories, performing and interpreting diagnostic tests, and recommending a plan of treatment. KEY RESPONSIBILITIES:
- Proven ability to perform comprehensive, adult/geriatric physical exam.
- Demonstrates ability to identify, diagnose and document chronic disease in adults.
- Demonstrates ability to recognize conditions, characteristics, and behaviors that place seniors at risk.
- Independently takes and reviews complete medical histories; performs appropriate physical examinations, assesses the patient and records pertinent data.
- Provides urgent medical patient care under an established set of protocols.
- Uses prudent judgement when emergencies require action beyond established protocols.
- Directs Primary and Urgent Care support staff when performing medical procedures.
- Orders, interprets and sometimes performs laboratory, radiology and other screening/diagnostic procedures.
- Performs routine therapeutic procedures, to include but not limited to injections; IV/IO starts;
fluid and medication infusions; immunizations; care of wounds/lacerations/bites/punctures/fractures/dislocations; management of trauma, cardiac and other emergencies; and all duties within the scope of PA training and certification.
- Instructs and counsels patients regarding matters pertaining to their physical and mental health.
- Refers patients to the appropriate health facilities, agencies and other resources in the
community. Stabilizes patients for transfer to higher level of care, when appropriate. Collaborates with local EMS providers when requested and appropriate.
- Orders, prescribes, administers and dispenses medications.
- Contributes to physician’s effectiveness by identifying short-term and long-range patient care issues that must be addressed; providing information and commentary pertinent to deliberations; recommending options and courses of action; implementing physician directives.
- Provides leadership and — when necessary – training to MA, LVN, RN support staff and visiting students and medical residents.
- Regular, reliable attendance is expected.
- Provides continuity of care by developing and implementing patient management plans.
- Maintains safe and clean working environment by complying with procedures, rules, and regulations.
- Protects patients and employees by adhering to infection-control policies and protocols.
- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
- Develops health care team staff by providing information, education opportunities, and experiential growth opportunities.
- Additional duties as assigned.
- Ensures all Members have an ICP developed within 48 hours of HRAT
- Facilitates agreement and implementation of the member’s plan of care by engaging beneficiary, the facility staff, families/responsible parties, primary and specialty care physicians
- Evaluates the effectiveness, necessity and efficiency of the plan, making revisions as needed
- Utilizes practice guidelines and protocols established by PruittHealth Premier Plan
- Participate with NP/PA interviews as requested
- Provide day to day supervision of assigned team nurse practitioners
- Develop on-call schedule for team
- Approve and manage PTO/leave to ensure sufficient levels of staffing coverage to meet the needs of the facilities
- Act as mentor and clinical resource for assigned team
- Offer training to help team NP increase knowledge and ability to apply research and evidence- based care to daily practice
- Participate in the performance evaluations for team, providing feedback to NPs and leadership as appropriate
- Collaborate with PHP leadership team, facility leadership, and primary care physicians to improve quality of care; and respond to PHP and CMS initiatives as indicated
- Assist with orientation of new-hire NPs
- Prioritize, organize and manage own workload in a manner that maintains and promotes quality;
and serve as an example to team members
- Other duties as need arises and as determined by leadership team
KNOWLEDGE, SKILLS, ABILITIES:
- Pertinent Clinical Work Experience which is relevant to population being served.
- Reviews and interprets the medical and social history of members
- Interprets laboratory data, recommend appropriate follow-up, reconcile medications
- Manages both medical and behavioral chronic and acute conditions effectively in collaboration with a physician or specialty provider
- Synthesizes information and easily diagnoses and creates a comprehensive plan of care.
- Ensures that individual plan of care is developed with specific interventions designed to meet the needs identified in the Health Risk Assessment
- Performs comprehensive assessments and document findings in a concise/comprehensive manner that is compliant with documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations
- Performs Health Risk Assessment within seven (7) days of enrollment for a new beneficiary and annually within one year of the initial assessment.
- Conducts an annual Comprehensive Geriatric Exam that incorporates CMS Annual a Wellness Visit elements, including the Personalized Prevention Plan Services (PPPS)
- Ensures all diagnoses are ICD-10 coded accurately and documented appropriately to support the diagnosis at that visit
- Conducts HRA comprehensive face-to-face assessment if the member experiences a change in condition and/or transition in care—a “trigger” event.
- Reviews orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the member’s needs and wishes
- Evaluates plan of care for cost effectiveness while meeting the needs of members, families and providers to decreases high costs, poor outcomes and unnecessary hospitalizations
- May be required to participate in on-call program, after hour or coverage may be required
- Rotating on call responsibilities
- Travels between care sites mandatory
- Performs other duties as assigned
MINIMUM EDUCATION REQUIRED: Graduate of an accredited Physician Assistant Studies, Health Science or Medical Science, Master’s Degree program. Previous long-term care experience strongly preferred. MINIMUM EXPERIENCE REQUIRED: Minimum 5 years clinical experience as a Physician Assistant in geriatric, adult or family practice setting. Proven ability to perform comprehensive, adult/geriatric physical exam. Ability to lead and manage other healthcare professionals, particularly NPs. Ability to use IT tools and programs including EMR, Microsoft Office Suite, Excel Excellent communication skills MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW: Current and unrestricted PA licensure in state of practice.
