Position Description
Mosaic Health Center is seeking a dynamic, team-oriented, patient-centered licensed Nurse Practitioner or Physician Assistant to serve in our community in a part time (20 hours a week) role. The position provides comprehensive, patient-centered medical care in an outpatient setting, diagnosing and treating acute and chronic conditions, promoting preventive care, and collaborating with an interdisciplinary team to improve patient outcomes. PRIMARY DUTIES AND RESPONSIBILITIES:
Duties include:
- Under the guidance of an MD, provide comprehensive primary care services including diagnosis, treatment, and management of acute and chronic conditions
- Perform routine physical exams and preventive screenings
- Manage chronic diseases such as diabetes, hypertension, asthma, and heart disease
- Provide evidence-based care aligned with clinical guidelines
- Order and interpret diagnostic tests and imaging
- Support monitoring and evaluation systems for the clinic’s programs and impact in health outcomes and social determinants of health
- Ensure consistent and efficient use of the EHR system for improved patient care, data collection and analysis, grant writing and quality improvement projects
- Promote preventive care, immunizations, and health education
- Address social determinants of health affecting patient outcomes
- Participate in population health initiatives and quality improvement programs
- Support care gap closure and health equity efforts
- Collaborate with nurses, behavioral health providers, care coordinators, and specialists
- Refer patients to specialty care and follow up on treatment plans
- Participate in care team meetings and case conferences
- Ensure continuity of care across settings
QUALIFICATIONS
A combination of education and experience providing the required skill and knowledge for success would require:
- Currently licensed as an advanced practice provider (either physician’s assistant or nurse practitioner) in the State of Georgia
- Two years of more of patient care as an APP
- DEA License preferred
- BLS, ACLS, certifications required
- EHR experience mandatory
KNOWLEDGE/SKILLS
- Knowledge of organizational policies, procedures, and systems
- Knowledge of clinic office procedures
- Knowledge of medical practices, terminology, and reimbursement policies
- Knowledge of Office Suite or equivalent applications or platforms
- Ability to read, interpret, and apply policies and procedures
- Ability to communicate clearly and effectively
- Ability to interact with patients, medical and administrative staff, public effectively
- Bilingual preferred: Hindi, Urdu, Gujarati, Spanish, Tigrinya, Amharic, Oromo, Arabic, etc.
SALARY RANGE: Competitive
Pay: $60.00 – $70.00 per hour
Experience:
- Patient Care as APRN: 2 years (Required)
License/Certification:
- certification for BLS and ACLS (Required)
- license as an advanced practice nurse in Georgia (Required)
