Diabetes Management Partners, LLC d/b/a GlyCare establishes contractual partnerships with Hospitals to provide dedicated inpatient diabetes and glycemic management teams. GlyCare’s methodology involves the translation of American Diabetes Association inpatient guidelines into clinical practice. These teams are spearheaded by nurse practitioners with specialized training in inpatient diabetes and glycemic management under the oversight of board-certified endocrinologists affiliated with GlyCare. Facilities partnered with GlyCare have seen reductions in the average length of stay, reductions in insulin-related errors and serious safety events, and improved average blood sugars across the facility’s hospitalized population.
We are seeking Nurse Practitioners and Physician Assistants to join our team! You will focus on providing clinically validated protocols tailored to each patient’s unique clinical condition. Experience in diabetes management is NOT necessary. All practitioners will undergo extensive training to diagnose, treat, and manage diabetes in the hospital.
Shift Type
Two days a week and every other weekend, which equates to an average of 12 days per month
Compensation
Compensation comprises a Base Salary and monthly RVU bonuses. Unlike traditional annual bonuses, our pay structure includes monthly increases based on actual census and case mix. This approach rewards employees in real-time for their performance and contributions, providing immediate recognition and motivation. We do not limit the dollar amount of monthly increases.
Base Salary = $70,000
Monthly RVU Bonuses = $3,500 to $7,500 (Annualized, paid monthly with no cap)
Addtl Comp and Benefits = $10,500
Total Annual Comp = $83,000 – $88,000
Qualifications
To be considered, you must be a licensed nurse practitioner in the state of Florida
New NPs – Clinical hours must have been in person at a facility/office
Medicare provider number is preferred but not required
Previous inpatient experience in nursing or other medical fields required
Experience as a nurse manager/supervisor or similar qualifications is a plus
A minimum one-year contract is required
The ability to thrive in a fast-paced environment is required
Must be detailed oriented
Excellent written and verbal communication skills
Ability to be efficient while managing your schedule without immediate supervision
Ability to quickly learn hospital electronic medical records software
Important
The hospital NO longer requires proof of C19 vaccine.
Experience in diabetes management is NOT necessary. GlyCare provides extensive training for inpatient diabetes and glycemic management.
We treat adult patients on Med-Surg and Step-down units.
NP specializations and concentrations that are a good fit include Family Practice (FNP), Acute Care (ACNP), and Adult Gerontology (AGNP).
Relocation assistance may be provided for qualified candidates.
The Details
Practitioners will learn how to treat and manage adult hospitalized patients with diabetes and patients with poor glycemic control.
Patients have clinical criteria for which a consult is automatically triggered.
You are expected to see 25 to 28 patients daily (typically 28-32 per day when the census is higher). These visits focus only on blood sugar control and management.
We start at 7:30 a.m. and leave when the last patient is seen. Depending on the number of providers rounding with you, the day typically ends between 2:30 and 4:30 (a 7—to 10-hour day).
PRN coverage is available and will be mandatory from time to time.
After-hours on-call responsibility is assigned on a rotating basis among all practitioners. The call can be taken from your home via phone and does not require you to return to the facility.
Monthly RVU bonuses are paid on Productivity, Volume, and Quality.
GlyCare covers 100% of the employee’s health insurance, with the employee responsible for additional coverage. Health insurance is a PPO plan through BlueCross

Two weeks of paid time off (PTO)
iPhone is required. A phone stipend of $600 is provided each year.
