Full Job Description
- This provider will work with patients remotely and in the field to assess and form appropriate treatment plans
- Communicates with the designated care team to maintain plan of care and share changes in patient conditions
- Mon-Fri: 8am – 5pm
- Field-based role
Required Qualifications for Nurse Practitioner / Physician Assistant:
- Licensed Nurse Practitioner OR Physician Assistant
- 1-2 Years of experience as an NP/PA
- Comfortable communicating with patients and care teams in-person and telephonically
Preferred Qualifications for Nurse Practitioner / Physician Assistant:
- Experience in Geriatrics, Primary Care, or Nephrology
